Main duties of the job

Key Responsibilities:

Conduct holistic health assessments using the Queen's Nursing Institute "Homeless Health Assessment tool."

Perform routine patient care tasks and baseline observations.

Support chronic disease management with a patient-centered approach.

Administer medications and perform clinical procedures as per training.

Collaborate with Salford Royal Hospital and local services to provide comprehensive care.

Advocate for patients, ensuring their rights, privacy, and inclusion.

About us

Salford Primary Care Together Inclusion Service is a specialist GP service dedicated to removing barriers to healthcare access for individuals without a fixed abode, including those rough sleeping, sofa surfing, or in emergency or temporary accommodations like hostels, hotels, and B&Bs.

Based in the clinical area upstairs in Salford Loaves and Fishes, this role also connects with the dynamic team downstairs who provide multiple services to the people of Salford.

Job responsibilities

Purpose of the role:

To provide and monitor care for people experiencing homelessness, working with Inclusion team and wider Salford homeless services to promote health and preventing ill health and integrated care. The job holder has a role in helping to create a culture at SPCT that values diversity and reflects inclusion.

Key responsibilities and duties of the role:

Patient Services, including planning and delivery

Utilise the Queen's Nursing Institute "Homeless Health Assessment tool" to make an accurate and holistic assessment of the health of homeless people.

Assist in and perform routine tasks related to patient care as directed by the Practice Nurse, GP or other member of the Primary HealthCare Team.

Be able to undertake baseline observations and identify signs of improvement, report any deteriorations or concerns, e.g. pulse, blood pressure, respirations, oxygen saturations, peak flow, spirometry, blood glucose levels, ECG, assessing nutrition and hydration, participate in screening programmes and Annual Reviews for example in patients with Learning Disabilities .

Support the Practice Nurse with the management of Inclusion patients with long-term conditions/chronic diseases, using a holistic patient centered approach.

Depending on qualification/training: Carry out Chronic Disease Management annual reviews delegated by the Practice Nurse or senior colleague.

Depending on training/qualification, perform cervical smear screening on completion of additional training

Depending on training/qualification, administer medications as prescribed and in accordance with NMC standards e.g. via nebuliser or injections using sub-cutaneous or intramuscular route under Patient Specific Directions.

Following agreed protocols with referral to Practice Nurse or GPs as appropriate assist in the assessment, planning, development implementation and evaluation of programmes to promote health and well-being and prevent adverse effects on health and well-being.

Deliver opportunistic health promotion using opportunities such as new patient medicals and NHS health checks.

Hospital Inreach: Collaborate with Salford Royal Hospital teams, including integrated Housing Officers, Ward personnel, and the Emergency Avoidance team, to offer guidance and clinical support regarding homelessness and inclusive health concerns.

Document consultations accurately and appropriately, according to NMC guidelines

Ability to recognise signs and symptoms of abuse/neglect and follow Safeguarding procedures

Ensure infection prevention and control guidelines are maintained.

To be responsible for providing the appropriate nursing evidenced based care of an allocated group of patients.

Be an accountable professional who is responsible for their actions, working in line with NMC standards of proficiency, recognising and work within the limits of their competency.

To work as part of multidisciplinary team using resources efficiently.

To deliver quality care to patients and to work effectively within the nursing team.

To be competent in carrying out a variety of clinical procedures.

To act as an advocate for the patient experiencing homelessness, including promoting and protecting their interests, privacy and rights, equality and inclusion.

Leadership and management, including coaching and developing team members

Where appropriate practice assessment of the non registered support workforce, TNAs, and pre-registration nursing graduates, paramedic graduates ect

Budget and reporting

To ensure medical and stationary supplies are stock checked and ordered as needed, seeking approval for budget sign off

To discuss and jointly apply (with service lead) for extra monies for additional service development/projects

Development of services

Contribute to and participate in programmes of audit, including utilising results in practice.

Where appropriate engage in projects to develop the service ie cancer screening drives, health workshops ect

Relationship management and development

Be responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with study requirements.

To communicate with the Practice Nurse and Service Lead and collect data in order to participate in the smooth running and organisation of the practice.

To establish and maintain effective communication, both verbally, written and relationships with the multi-disciplinary team.

To recognise, prevent where possible, and manage areas of conflict, reporting to the Practice Manager if unable to resolve.

Participate in regular practice supervision

Participate annually identifying, developing and agreeing your own development plan with your line manager through the appraisal process.

Maintain professional conduct including appearance at all times

  • NVQ 2 or 3 in Health and Social Care or Equivalent
