Salford Primary Care Together (SPCT) Practices are proud to offer an exciting opportunity for a practice nurse to work in an Outstanding GP practice (CQC May 2019). We aim to be trailblazers and as part of an excellent multi-disciplinary team, our Clinicians provide fantastic care through teamwork and peer support. We currently serve a population of circa 20,000 people across our 3 core sites and specialist Inclusion Health Service.

Main duties of the job

The ideal candidate will be a nurse who is ready to challenge the norm in Primary Care, as part of a forward-thinking team of clinical and non-clinical staff.

About us

SPCT Practices are uniquely placed as part of the wider SPCT organization, which currently also runs a variety of Salford-wide primary care services. SPCT Practices also runs the Inclusion Health Service, a specialist primary care provider for people that fall through the gaps in regular Primary Care. Within the Inclusion Service we have a well-established and growing Homeless service delivering specialist clinics plus in-reach and outreach services. SPCT are running an innovation project for care leavers though the Inclusion Service and soon hope to be delivering an asylum seeker service too.

RESPONSIBLE TO: Lead Practice Nurse

ACCCOUNTABLE TO: Clinical lead

JOB SUMMARY: To contribute to the successful delivery of the key outcomes of the SPCT business plan by provision of a high quality nursing service to a defined practice population. This will include assessment, health promotion, chronic disease management, health screening and treatment in line with the professional competence of the post holder. To be involved with the practice delivery of National Service Framework targets and local Health Improvement priorities. Identification of nursing needs of the population. Responsibilities to maintaining and improving QOF data in line with the business plan and Salford Primary Care Togethers objectives.
