Job summary

SPCT Practices is a friendly multi-site Practice serving a population of around 20,000 people across 3 sites in Salford and a specialist Inclusion Health service. We are part of the wider Salford Primary Care Together organisation, a provider organisation for Priamry Care in Salford that delivers a range of city-wide services including GP OOH and Covid services through the Enhanced Primary Integrated Care (EPIC) team. SPCT Practices core sites are based in Eccles, Little Hulton and Lower Kersal.

Main duties of the job

We are looking for a Lead Nurse to join SPCT Practices to lead our fantastic team of practice nurses and health care assistants across all of our sites. The successful candidate will support and contribute to the development our nursing team to promote high quality and compassionate nursing care and to champion the best chronic disease management.

SPCT Practices aspire to be trailblazers in Primary Care and have a track record of innovation and quality improvement. In addition to providing practice nursing care, the Lead Practice Nurse will be responsible for managing and supporting the development of our team of 5 practice nurses , 3 health care assistants, 1 assistant practitioner and 1 phlebotomist with dedicated management time to do this in.

Job responsibilities

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population. As the team leader for the nursing team, the post holder is accountable for nursing service delivery. They will lead and manage all the nursing resource, working closely with the practice management team and GPs to deliver the practice priorities. Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership and direction for the nursing team.

  • Community nursing specialist qualification or equivalent
  • Relevant nursing/health degree
  • Mentor/teaching qualification
  • Clinical supervision training and experience

Desirable

  • Independent/supplementary nurse prescribing qualification

  • Minimum 5 years post registration experience
  • At least 2 years recent primary and community nursing experience
  • Experience in nurse-led management of long-term conditions
