Job summary

We are seeking an innovative, creative and accomplished leader to join us to lead and support GP colleagues in our Primary Care Networks to deliver their mandate and ensure high quality outcomes for the patients of Gateshead. 

You will lead a small dedicated team in the delivery of outcome focussed goals using your leadership and empowering skills to overcome challenges and perceived barriers. This post will form part of CBC's senior management team and the successful applicant must be aligned to CBC's values and behaviours as they will work alongside the other divisional managers and CEO.

You will be an accomplished leader and manager with high level of communication skills, preferably from a health or social care background. If you are looking or a new challenge and want to join a growing forward-thinking company then you are the person, we are looking for.

Main duties of the job

The post is to lead a small team of staff in the support and continuous development of Primary Care Networks and general practice across Gateshead. The post sits within CBC Health, as a provider of primary care support and services for general practice in Gateshead. 

The post holder will be responsible for the planning, development, delivery, and accountability of an agreed portfolio of work as part of CBC’s endeavour to best support general practice, but also assist the Gateshead PCN’s in their transformation and change agenda. 

The post holder must be organised, capable of systematic planning but also able to be flexible to changing demands in line with local or National directives. The post holder will be nominally based at CBC office at Queens Park Team Valley but will be expected to travel across Gateshead to individual practices or PCN meetings in addition to attending CBC senior management forums. 

This is senior post within CBC and will be accountable to the clinical lead for the Primary Care Collaboration Team, and a direct report of the CEO of CBC.

MAIN DUTIES AND RESPONSIBILITIES

Responsible and accountable for the delivery of a defined portfolio of work from the Primary Care collaboration team and PCN’s in Gateshead 

Support and deliver integrated working with Gateshead PCNs, and system delivery in line with local and National agendas 

Co-ordinate CBC’s support offer to PCNs and practices 

Support and contribute to work within CBC and PCNs to reduce Health Inequalities 

Support internally to CBC as a senior manager taking responsibility for defined portfolio of work agreed with CEO/ Director 

Leadership of a defined team including providing all line management responsibilities 

Production, delivery, and evaluation of projects and programs of work 

Engagement and excellent communication with immediate teams, CBC staff and senior managers, PCNs and other stakeholders to ensure integration with PCCT and PCN work portfolio

Development of high-quality business cases

Source opportunities for business development and expansion of services within the PCCT PCN’s, and CBC 

Actively seek business development opportunities for CBC

Preparation of board papers in line with CBC policies and procedures 

Produce a variety of documents to a high standard with appropriate levels of detail for the designated audience 

Support the Clinical Lead of the PCCT in their portfolio of work 

Act as an advocate for CBC and the PCN’s representing at appropriate meetings and forums as and when required

To develop, in conjunction with the PCNs, Gateshead Virtual Team, CCG, and Leadership team, performance metrics and measures for the project, and to work with the team to implement these successfully 

To lead on and project manage aspects of the overall project, as required 

To work closely with PCN CDs, Leadership Team on the development, resourcing and implementation of clinical workstreams 

To ensure the development of models are based on best evidence, research and practice, which may be applied in Gateshead 

To assist in the delivery of the identified key project areas with accurate and timely reporting including financial management, business case preparation and metric delivery within the GP Federation and potentially within CBC Health Strategy, Planning and Local Context 

To work with the CBC Leadership Team, PCNs and Practices in the development of a strategic plan for the Primary Care collaboration team, and to ensure supporting business plans are in place and outcomes are achieved 

To ensure these plans fit with the strategic development of the ICS in Gateshead and are a good strategic fit with the overall strategic plans in Gateshead, and in line with national thinking 

To work into the ICS developments on behalf of CBC as required 

To support CBC in its overarching business objectives with clears targets and delivery schedules Financial Management 

To manage and be accountable for the budget for the PCCT 

To ensure the Leadership Team has up to date financial information, working with the Finance Manager, CBC Health, and PCCT to deliver this 

To develop an expertise in terms of financial issues relevant to General Practice, and to work closely with external contracted finance experts in assessing the impact on local GPs and looking for innovative ways of supporting individual practices and PPCN’s to get the most from the “Gateshead pound”
