Main duties of the job
As a Specialist Medicines Optimisation Pharmacist, you will work closely with a variety of healthcare professionals across Gateshead, Newcastle, and South Tyneside, offering expert advice on medications and treatments. The successful candidates will be supported by an experienced team of pharmacists and pharmacy technicians, working collaboratively across the region. This role includes becoming involved in the following projects:
- Chronic Kidney Disease (CKD) Project (Pilot) You will be involved in a cutting-edge project aimed at the early identification and diagnosis of Chronic Kidney Disease (CKD). Working across selected GP practices in Gateshead, you will review patient records, interpret diagnostic results, and lead patient-facing consultations. The goal is to optimise the management of CKD, reduce disease progression, and improve long-term patient outcomes. You will work closely with GP practices, secondary care colleagues, and the wider project team.
- Care Home Pharmacist South Tyneside In this role, you will provide vital support to care homes across South Tyneside. You will liaise directly with care home staff, GPs, nurses, and pharmacy colleagues to deliver medicines reconciliation and ensure the safe and effective use of medications for residents. Your input will be instrumental in advising healthcare professionals and ensuring the ongoing safety and wellbeing of care home residents.

- Experience of working in primary care
- Specialist knowledge acquired through clinical diploma or equivalent experience
- In-depth knowledge of therapeutics, especially medicines used in primary care
- Has awareness of the common acute and long-term conditions that are likely to be seen in general practice
- Excellent verbal and written communication skills
- IT Literate
- Evidence of an ability to build successful working relationships with a range of professionals and administrative staff
- Good problem-solving skills and the ability to make judgements
- Time management
- Professional and discreet
- Adaptable and flexible with the ability to work to tight deadlines and manage competing demands
- Experience of training and supporting less experienced staff
- High degree of concentration with potential for frequent interruption
