Job summary
We are looking for an experienced dietitian who is happy to
travel to various facilities within our services, to provide dietetic support
and catering involvement. Our services include complex older person mental
health and secure services in Learning Disability/Autism.
We also have
contracts for specific dietetic needs at our College in Vale of Glamorgan.
This is an exciting clinical post that requires the
applicant to work independently whilst receiving the full support of the multi
professional teams on each site.
We actively promote training and
supervision on a regular basis and ensure the post holder is an integrated part
of local governance and audit process. We pay HCPC registration fees as well as
offering a variety of resources to support individual wellness.
Main duties of the job
To manage a specialist case load of patients
with complex nutritional needs, using evidence based and person-centred
principles, to assess, plan, implement and evaluate treatment.
To manage all service resources
effectively.
To take
responsibility as an autonomous practitioner in planning and developing a
dietetic service with a range of adults with complex needs in older persons
mental health/ mental health and learning disability

Clinical
- Complete full assessment of patients nutritional status, using appropriate anthropometric, biochemical and medical records and identify their nutritional requirements.
- Devise core and specialist nutritional treatment plans e.g. diabetes, weight reducing, food intolerance, enteral nutrition, renal nutrition, disordered eating.
- Implement specialist group and individual treatments to achieve goals.
- Use patient centred motivation techniques to effect nutritional behaviour change.
- Evaluate and modify interventions using appropriate outcome measures.
- Practice in accordance with legislation according to your clinical area.
Communication
Communicate with patients, using the most appropriate verbal, written or pictorial method, in a way that respects their view, autonomy and culture.
- Translate key nutritional messages into simple ideas and language that can be easily understood by the client group.
- Work collaboratively with all members of the clinical team and share information with the team members formally and informally through morning meetings/huddles/CTP and CPAs and MDT notes.
- Liaise with other departments within the service and beyond.
