Job summary
An exciting opportunity has arisen for a clinically focused, highly motivated clinical pharmacist who wishes to expand their experience by working remotelywithin a number of General Practices across the Broughton area within the PCN.
Main duties of the job
As a Clinical Pharmacist you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists. You will work as part of a multi-disciplinary team working with colleagues to help resolve medication-related issues, helping to utilise skill mix, improve patient outcomes and ensure better access to healthcare.
You will support practice staff with prescription and medication queries. Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, proving expertise in clinical medicines advice while addressing both public and social care needs of patients.
The role is pivotal to improving quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

You will be enrolled on an 18-month national training program, including attainment of an independent prescribing qualification (if applicable). You will also be supported by a national network of pharmacists who are undertaking the same journey
