Job summary

The Pharmacist Network are currently hiring for an exciting remote opportunity. We are looking for highly motivated candidate to support with day-to-day activities for Central London PCN in Central London.

If you are experienced in working in a General Practice (essential) we'd like to hear from you. This is a on site role suitable for candidates looking for on site permanent work.

Main duties of the job

Using systems (emis and s1) you will support the Central London site with medicines related tasks, this may include but is not limited to:

- discharge support (meds rec)

- general med reviews

- supporting patient requests for acute and repeat meds

- Structured Medication reviews

- Safety audits

You will be proficient in the use of GP systems. You will be an excellent communicator with a positive and proactive outlook welcome.

You will work as part of a multi-disciplinary team in a patient-facing role, working with colleagues to help resolve medication-related issues, helping to utilise skill mix, improve patient outcomes and ensure better access to healthcare.

You will support practice staff with prescription and medication queries. Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.
