Job summary
This
role is 24 hours per week, based across the geographical area of Mid-Norfolk and serves the
eligible population of the Mid-Norfolk Practices.
As part of a
multi-disciplinary team, the Acute Visiting Team provides direct clinical management
of patients with acute healthcare needs, living in care homes and in their own
homes within the locality.
This role will be ideally suited to a registered
healthcare practitioner from a nursing or paramedic background who holds an
independent prescribing qualification and is experienced in providing care to
this patient group.
Main duties of the job
Joining the existing team, the post
holder will provide advanced clinical skills to support General Practice in the
management of frail and elderly patients through acute home visits. They will also participate in a programme of
clinical audit, research and quality initiative and provide support and
supervision to more junior members of the team.
Clinically they will provide direct care to patients by undertaking a
comprehensive history, clinical assessment, treatment, and evaluation of
patients care. The post holder will
work collaboratively with the multi-disciplinary practice teams and the wider
Integrated Care team to meet the needs of patients and help to avoid
unnecessary admissions to hospital.

- - Minimum of 2 years past experience in General Practice or community care settings
- - Can demonstrate competency in autonomous practice
- - Evidence of completed clinical audit
- - IT literate
- - Up to date knowledge of national and local policies, including national service framework and relevant NICE guidance
- - Ability to work effectively in a team and independently
- - Understanding of evidence-based practice
Desirable
- - Demonstrates ability in teaching, research, audit and management
- - Experience of setting customer care and patient participation as a priority
