Job summary

Mid Norfolk Primary Care Network (PCN) are looking to recruit a PCN Care Coordinator to work collaboratively across the Practices and linked care homes to provide administrative support to clinical teams and other members of the PCN.

Main duties of the job

To enhance our teams, we are looking for highly motivated individuals to work with our local care homes to bring together all parties involved in the care of the patients registered with our member GP Practices.

The Care Coordinator role is seen as an evolving post to support the Multi-Disciplinary Teams (MDTs) within the PCNs to deliver effective, co-ordinated care for patients in care homes.

Job responsibilities

A PCN care co-ordinator may be required to deal with patients and, if appropriate, their carer, before or after the patients consultation with a clinician or other healthcare professional.

Working closely with the patient and their clinician or other healthcare professional, the care coordinator co-ordinates patients healthcare and directs them to the appropriate service to ensure that they get the most suitable care from whatever health or social care provider is appropriate.

The role requires you to be able to work with, and understand the roles of, a variety of different people working in the practice and across the PCN including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician associates, paramedics, health and wellbeing coaches, podiatrists, occupational therapists and pharmacy technicians.

They may be given a caseload of identified patients and be required to ensure that their changing needs are addressed by considering local priorities, health inequalities and/or population health management risk stratification.

  • A-level / NVQ 3 or equivalent experience in admin / business / marketing / customer service environment

  • Excellent working knowledge of Microsoft Office software
  • Experience of working in a healthcare setting
  • Experience of office procedures working at a high level as part of an administration team / within an administration role
  • Experience of dealing with sensitive/confidential information
  • Proven administrative experience
  • Understanding of Confidentiality and Data Protection Act
  • Working knowledge of the management of databases
  • Understanding and able to deal with confidential and sensitive issues when liaising with team members / other professionals
  • Ability to prioritise and organise workload to meet deadlines
  • Ability to work under pressure with constant interruptions requiring skills in multi-tasking, maintaining accuracy at all times
  • Ability to problem solve and support others in resolving problems
  • Ability to work in partnership with other agencies
  • Ability to manage conflicting issues assertively and sensitively
