Job summary

You will join our supportive multidisciplinary team of doctors, nurses, pharmacists, therapists and social workers to improve the quality of care we offer by supporting the practices to work more closely with our local care homes and their patients. You will help us to coordinate the administration and implementation of a key project required by the NHS to help care home patients live well and help clinicians look after them when they reach the end of their lives. The successful candidates will work together across the PCN to support the needs of practices, care homes and patients. Flexible, part-time and job share roles are considered and actively supported.

These are flexible roles as you will be contributing to work that will benefit the PCN as a whole and also specific and targeted work with individual Practices; we are therefore looking for individuals who are motivated and have a desire to deliver an excellent service and support.

About us

Mid Norfolk Primary Care Network (PCN) is a group of 6 GP surgeries in and around Dereham in Mid Norfolk. We have worked closely together as a collaborative group for several years and are now looking to recruit 2 whole time equivalent Care Co-ordinators. South Norfolk Health care as the GP provider organisation is working to support and develop the PCN's.

The opportunity for two whole time equivalent roles which can be flexible in terms of how these are fulfilled. Each care coordinator however will support and be based at either one of two of the practices within the PCN and not be expected to cover them all.

Role Overview

Mid Norfolk Primary Care Network are looking to recruit two PCN Care Coordinators to work collaboratively across the Practices and linked care homes to provide administrative support to clinical teams and other members of the PCN.

To enhance our teams, we are looking for highly motivated individuals to work with our surgeries and local care homes to bring together all parties involved in the care of our registered patients. The Care Coordinator role is seen as an evolving post to support the Multi-Disciplinary Teams (MDTs) within the PCNs to deliver effective, co-ordinated care for patients in both their own homes and those residing in our linked care homes.

A PCN care co-ordinator may be required to deal with patients and, if appropriate, their carers, before or after the patients consultation with a clinician or other healthcare professional.

Working closely with the patient and healthcare professionals, the care coordinator, co-ordinates patients healthcare and directs them to the appropriate service to ensure that they get the most suitable care from whatever health or social care provider is appropriate.

The role requires you to be able to work with, and understand the roles of, a variety of different people working in the practice and across the PCN including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician associates, paramedics, health and wellbeing coaches, podiatrists, occupational therapists and pharmacy technicians.

They may be given a caseload of identified patients and be required to ensure that their changing needs are addressed by considering local priorities, health inequalities and/or population health management risk stratification.

Main Duties and Responsibilities

The post holders, working closely with the PCN team, care homes, GP Practices and existing community services will:

Work collaboratively with other teams and services to maintain an effective and efficient service.

Plan and organise work using own initiative, whilst being able to work as a valuable team member.

Be able to make both routine and urgent referrals to relevant service providers including the relevant safeguarding and admission avoidance teams.

Coordinate and where required chair a variety of MDT meetings at the surgeries, having proactively prepared any actions prior to the meeting and ensure all relevant clinical and non-clinical staff are present.

To promote and facilitate integration within the GP and community teams.

To provide and always receive sensitive information about difficult or complex matters respecting confidentiality.

Create detailed and clear minutes from the meetings and forward to all relevant parties with actions highlighted to individuals.

Where required, be able to offer appropriate support and guidance to patients and their families / carers.

Receive, record and collate information and maintain accurate electronic records of patient care and planned service provision, including hospital admission and discharge information.

Signpost service users and carers to relevant services.

Contribute to quality audits.

Cooperate and support colleagues to share best practice, complete tasks to maximise effectiveness and team performance.

Assist with preparation of ReSPECT forms where required.

Take part in appropriate training and take responsibility for own personal development.

Contribute to the development of the role and systems, to effectively enhance the service provided and improve outcomes.

To assist patients to be better prepared to have conversations on shared decision making and to improve awareness of shared decision making and related support tools

To provide patients with high quality, easy to understand information to assist them in making choices about their care

It is anticipated that the postholder may also be asked to support registered patients outside the care home setting where required

To support in the delivery of enhanced services and other service requirements on behalf of the PCN

Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives
