Job summary
GP Care Group is recruiting for a Community Nursery Nurse (CNN) to
join the Infant Feeding Team. This role is part of a collaborative project in
partnership with the 0-19 Public Health Nursing Services and HENRY, focused on
supporting mothers and families with infant feeding.
This position offers a unique opportunity to make a tangible
impact on the health and wellbeing of families in Waltham Forest, either
through a secondment or as a new role. You'll be part of a committed team
dedicated to delivering high-quality, personalised support to parents during a
crucial period of their childs development.
This role offers excellent opportunities for development and

feeding, early years care, and public health. It provides scope for contributing to a pioneering model of care, with
support from experienced colleagues and partner organisations.
This is a fixed term contract that ends March 2026.
Hours are 30 hour per week worked across 5 days (Monday to Friday)
Main duties of the job
The postholder will:
- Be a core member of the Infant Feeding Team within GP Care Group.
- Support mothers with breastfeeding, responsive feeding, and informed feeding choices.
- Contact all mothers/parents/caregivers between 35 days post-birth to provide infant feeding advice and relevant health promotion.
- Escalate concerns to the Health Visitor for targeted assessment when appropriate.
- Provide follow-up support through targeted home visits, in Family Hubs, or in other community settings preferred by the family.
- Recognise developmental stages and intervene where challenges arise, empowering families through tailored support.
- Work collaboratively within the multi-disciplinary 019 team, offering holistic support around infant care and parental wellbeing.
- Deliver interventions under the direction of the Infant Feeding Lead, both antenatally and postnatally, in accordance with the local Integrated Infant Feeding Pathway.
This is a community-based role, providing support and interventions where needed and making a meaningful contribution to early childhood development in Waltham Forest.

- Contact mother/Caregivers 3-5 days postnatally to advise of feeding choices/support and support forwell-being.
- Provide Infant support across Healthy Child Clinics/Family Hubs.
- Provide 121 support under delegation of named cased holder.
- Undertake delegated Health reviews for Children .
- Report back to the appropriate team member any significant changes in the client circumstances that may impact on safety of others.
- To establish and maintain communications with parents, children and other professionals in a varietyof settings and mediums about daily activities, child care and child related information.
- Overcome barriers to communications such as language or learning difficulties.
- Provide empathy and reassurance between persons involved within scope of the role.
- Develop relationships with professionals groups including but not limited to; Midwives, HealthVisitors, GPs, Childrens centres and mental health services.
- To be proactive in liaising with non-statutory/voluntary/community based agencies
- To provide information and participation in case conferences and reports relating to specific cases

Planning and Organisational Skills:
- Plan and manage own workload, and contribute to organisation of the team and the service
- Work in line with the Lone Working Policy and highlight any health and safety concerns as required.
- To be responsible for own activity data reporting.
- Organise own time.
- Prioritise service user care under the supervision of qualified perinatal practitioners.
- Under the supervision of registered practitioners, making decisions about service user care asappropriate, with the support of the wider multi-disciplinary perinatal team.

- Work within the Local Safeguarding Board guidelines for child protection, including attendance atclient related case conferences where necessary to discuss your observations following work withthe family.
- To access child protection supervision.
- To maintain accurate and contemporaneous electronic records of work undertaken.
- Ensure strict confidentiality in respect of clients and staff.
- Provide practical and emotional support to help women prepare for the transition into parenthood.
- Supporting partners, siblings and wider family members prior to arrival of the baby.
- Enhancing attachment and bonding through pregnancy using current research.
- Help to support families understand the world from the babys perspective.
- Explore the demand and need to develop and facilitate groups to encourage socialisation andsignposting to childrens centres/Family Hubs.
