An exciting opportunity exists for progressive forward-thinking ACPs to join our high performing enthusiastic team in the Urgent Treatment Centre collocated to the Emergency Department at the Royal London Hospital site. Main duties of the job

The post holder is an experienced clinician who, acting within their professional boundaries, will work flexibly and collaboratively with other members of the UTC team to provide care for the presenting-patient, from initial history taking, clinical assessment, diagnosis, treatment to evaluation of care within the urgent care setting.

The post holder will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills to manage patients out of hospital taking NHS 111 Direct booking calls to patients streamed to UTC.

The post holder will understand complexity, demonstrate critical thinking, in the clinical decision-making process whilst supporting the delivery of policy and procedures, providing clinical leadership as required in line with the 4 pillars of advanced practice.

Job description

Job responsibilities

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undiagnosed and undifferentiated illness and injury (children and adults) often with complexity.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.
  • Arrange and interpret investigations within the remit of the Urgent treatment service such as blood tests and X-rays.
  • Acts as an autonomous practitioner within agreed protocols guided by organisational policies and within Scope of Professional Practice and governing body.
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and local protocols, and within scope of practice.
  • Work with patients in order to support compliance with and adherence to prescribed treatments.
  • Adapt to clinical services which may be new, innovative and challenging at times with a flexible approach to learning the skills required to perform optimally in the role.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
