Job summary
Tower Hamlets GP Care
Group is a federation of 30+ General Practices which cares for more than
312,000 people. Network One is a primary care network (PCN) of four GP practices around Bethnal
Green in Tower Hamlets, East London, that serves around 50,000 patients in the borough.
The PCN is looking to recruit three GP Assistants predominantly to support practices with their domiciliary phlebotomy.

Main duties of the job
As a PCN GP Assistant you will support smooth running of clinics by performing routine administration and clinical tasks. You will work closely with practice GPs and other staff in order to streamline the work of the practice. You will be a front line worker and will undertake training to do additional responsibilities such as those routinely undertaken by Health Care Assistants, including home visits for domiciliary phlebotomy. You will work closely with the clinical lead for the PCN Care Home, assisting with the duties therein, including performing the weekly care home round.
The
PCN has a growing team consisting of Occupational Therapists, Clinical
Pharmacists, Care Coordinators, Social Prescribers, Health and Wellbeing
Coaches, GP Assistants and more, who work together to deliver enhanced care to
the Networks population.

Job responsibilities
- Take and record brief medical history.
- Accurately record patient notes in the clinical system, including
accurate use of SNOMED codes and GPAD slot types.
- Take patient height, weight and BMI.
- Be able to act as a chaperone following chaperone training.
- Explain treatment procedures to patients.
- Be able to monitor vaccine storage and record fridge temperatures.
- Refer patients to the appropriate members of the team e.g. Social
Prescriber / Health Coach for loneliness, Pharmacist for medication review, GP
for loss of appetite.
- Perform phlebotomy both on premises and in a domiciliary setting.
- Be able to take ECGs, blood pressure, pulse check, dipping urine, and
other diagnostic tests following appropriate training.
- Support with wound care and immunisations.
- With completion of care certificate:
- Facilitate 24 hour BP Monitoring
- INR Monitoring
- Offer life style advice
- Administer B12 and flu injections
- Complete new patient health checks, NHS Health Checks and smoking
cessation (with on-ward referral as necessary to GP/Health Coach/Third Party
Service)
- Spirometry and Reversibility Testing
- Chronic Disease Management: Diabetes Foot Check and Annual Review,
