We are one of 5 sites across the country who are involved in Start 4 Life Workforce pilot. One of the pathways in Tower Hamlets is immunisation plus aimed at families who have children in the First 1001 days.
We are excited to offer experienced nurses the opportunity to be part of this pilot. We are running immunisation information clinics to parents and carers living in Tower Hamlets. The uniqueness of this pilot is we are offering to deliver baby vaccines in different venues such as children centres while working closely with our GP partners. The staff can also discuss other issues that may be worrying parents such as infant feeding, starting solids or their own health. All appointments are conducted in a confidential area.
Staff will be trained in the Practice Elements which includes motivational interviewing techniques.

Main duties of the job
The post holder will have a delegated caseload under supervision Lead Immunisation Nurse and other senior staff members. The post holder will have a responsibility to carry out specific nursing care for patients, families/ carers. The post holder will work in partnership with the Multi-Disciplinary Team, and other agencies, to ensure that excellent standards of care are provided.
The 0-19 Universal Service teams provides support to all children and young people and their families within a progressive universal model of service delivery dependant on assessed need. The service offers every child/family a programme of screening, immunisations, developmental reviews and information and guidance to support parenting and healthy choices as defined by The Healthy Child Programme (Department of Health, 2011).
The GP Care Group 0-19 Universal teams provide a safe, effective, and high-quality service to clients using research-based evidence to inform practice within agreed professional standards.
The 0-19 Universal Service Team Community Staff Nurse will support the delivery of the Healthy Child Programme within a universal model of service delivery dependant on assessed needs and provision of personalised care.
This role will play a key part in the development and safe delivery of 0-19 services, utilising best available evidence to improve the health and wellbeing of children and young people especially those from disadvantaged groups.

- To deliver Childhood immunisations as directed by appropriate governance routes as designated by the service offer, and overseen by the GPCG, and Lead Pilot pharmacist.
- Provide ongoing holistic care and in line with scope of practice for the delivery of the Healthy Child programme high impact areas and schedule of interventions.
- To develop working relationships with families and work undersection and delegation as required of the caseload holder, delivering consistent messages to include infant feeding support, emotional well-being, signposting across the wider 0-19 and Family hub offer.
- To support and assist Assistant practitioners for speech and Language and 0-19 Universal Community Nursery Nurses as required.
- Apply a Make every Contact Count (MECC) approach in service delivery.
- To participate in Health Education programmes and health promotion activities on a multi-disciplinary basis across the 0-19 partnership and Family Hub Offer.
