We are launching our Quality agenda and are very excited to be recruiting to our team of Specialist SCPHN Nurses (Health Visitors and School Nurses) who have are key role in supporting front-line teams to provide a high quality and integrated care and support to parents and babies.

The team will have a key role in ensuring the full Healthy Child Programme and High Impact Areas are delivered and contribute to addressing health inequalities, working closely with our Family Hub/GP Practices/community children's service and 3rd Sector to deliver integrated services that are innovative, progressive and responsive to the needs of Tower Hamlets community.

You'll be part of a dedicated team focused on making a tangible difference in the lives of families in Tower Hamlets.

Main duties of the job

The Specialist Practitioner will work with other services within Health, Social Care, Primary Care and across the Family Hub partnership to develop /strengthen evidence based perinatal pathways, training, policies, and process to clearly provide direction in relation to assessment and evaluation of care and ongoing support at the earliest opportunity.

Aim of the role:

To be responsible for leading Perinatal Mental and Infant Mental Health across the Tower Hamlets 0-19 Service.

To drive forward new ways of working, supporting management to ensure developments areprogressed.

The Specialist Practitioner will work with other services within Health, Social Care and across theFamily Hub partnership to develop /strengthen evidence based perinatal pathways, training, policies,and process to clearly provide direction in relation to assessment and evaluation of care and ongoingsupport at the earliest opportunity.

The specialist will be a strategic thinker and be proactivelyinvolved in operational developments and support with clinical expertise and service strategicdevelopments.

To provide specialist perinatal mental health advice, support and training for clinicians and otherrelevant workers within the GP Care Group 0-19 Team, and across the Family Hubs partnershipCaseload 40%.

Key Responsibilities

  • Provide leadership and embed research and evidence-based practice to enable safe and quality provision of care in relation to emotional well-being, perinatal and Infant mental Health.
  • Provide specialist Clinical Leadership for practitioners, and expertise and direction within Family Hubs.
  • To support practitioners in managing complex community health assessments.
  • To support practitioners in their assessments if required of effectiveness of a clinical intervention via direct support, supervision, action learning, reflection.
  • To lead met of protocols, policies, and clinical guidelines/pathways in relation to Perinatal and Infant mental health.
  • Coordinate the National and local Training programmes across all tiers across the Family Hub services. This will include working with identified trainers for training delivery to ensure all staff have received the appropriate training offer in line with their scope of practice.
  • To deliver comprehensive training and education for staff within universal services and partner agencies in the early identification and support of women suffering with postnatal depression and its effects on childrens mental health and behaviour.
  • To work collaboratively across organisations and organisational boundaries to ensure the delivery of high quality effective perinatal mental health services.
  • To provide specialist advice and evidenced based information to staff to assist them to lead and help clients make more informed choices, but also enable effective care pathways that safeguard child and parent/s.
  • Develop outcome focussed services.
  • Develop robust links with the Health Visiting Advanced Practitioners for Behaviour Management and the Primary Mental Health Workers.
  • To act as clinical champion in re-shaping the wider 0-19 Service.
  • Responsible for sharing insights and best practices at learning forums, fostering a culture of continuous learning and improvement within the 0-19 service.
  • To lead and participate in area relating the 0-19 service development.
  • Provide a link between 0-19 colleagues and Specialist Services in relation to families where there are challenges in relational to perinatal and Infant mental health.
  • To promote and as required provide high quality clinical intervention with specialist arena, such use of Video Intervention group activity.
  • To promote and provide high quality clinical intervention for allocated clinical case management.
  • To provide supervision in relation to specialist field to staff as required.
  • Gather user feedback to continuously improve service delivery. This will involve organising and facilitating focus groups with families in partnership with services in the designated Borough promoting integration to enable access to all required services in a timely and seamless manner.
  • Ensure 0-19 service adherence to National standards such as NICE and Healthy Child Programme High impact areas, via development of clear pathways and actions plans as required.
  • To coordinate training across the Family Hubs, offering clear direction and training for facilitators/trainers to roll out service specific training.
  • To undertake training needs analysis, supporting commission and evaluation of service and Family Hub training programmes.
  • To work closely with ante natal and postnatal services in Family Hubs in order to reduce the incidence of perinatal depression and its impact on childrens mental health and behaviour.
  • To develop care pathways in line with National direction and local provision with all agencies to enhance the clients experience.
  • To be responsible for ensuring that accurate information is collected, analysed and presented to ensure effective use of resources and inform future service planning.
