Job summary
We seek 2 full time and 1 fixed term full time (1 year) Community Nursery Nurse.

The post holder will work as a member of the
Integrated 0 19 Service Health Visiting and School Health and Wellbeing and
in collaboration with Childrens Centres, Early Years Providers, GPs, school,
and education staff to support planned programmes of health care and health
promotion, within clients homes, Childrens Centres, GPs surgeries and
primary schools within the Borough of Tower Hamlets.
As a valued member of the health visiting team, you will join a
friendly and supportive team, and will undertake child health reviews, health
screening procedures including the National Child Measurement Programme (NCMP),
vision and hearing and contribute to individual and group health promotion
activities. You will be expected to
provide specific interventions with individual children according to care plans
drawn up by the Health Visitor or School Nurse and in conjunction with parents
and school staff.
In

Main duties of the job
The Nursery Nurses within the Integrated 0 19 Service Health Visiting
and School Health and Wellbeing are responsible for the delivery of a range of
programmes that support the outcomes identified with the Health Child Programme
0 19. This will include the delivery
of the NCMP, vision and hearing screening with Year Reception and Year 6 pupils. Also, the delivery of health promotion mainly
within Childrens Centres and primary school setting.

Duties and responsibilities of this role includes:
- Manage, plan and implement the delivery of
the NCMP, vision and hearing screening for identified cohort of schools in a
timely manner to ensure compliance with national screening timeframe.
- Support the delivery of child health clinics.
- Undertake delegated developmental reviews and
report back to Health Visitor/School Nurse.
- Making an assessment of each childs health
in the cohort of school children in the designated primary schools, according
to service guidelines and with support and advise from the School Nurse using
Parent/carer questionnaires, teacher questionnaires and Health Visiting records.
- To review Health Visiting records of cohorts
of primary school children and to refer concerns to the School Nurse.
- To review questionnaires from parents/carers
and to check consent for child health screening tests. To refer concerns to
School Nurse.
- To review questionnaires from class teachers
regarding health of school children in designated primary schools and to refer
concerns to the School Nurse.

To carry out delegated health assessments
including interviews and report any concerns to the Health Visitor/School Nurse.
- To carry out delegated health screening of
school children in primary schools in accordance with organisational policies,
procedures and guidelines.
- To identify and refer to the Health Visitor/School
Nurse any concerns arising from screening procedures or highlighted by parents
or school staff.
- To make referrals to other professionals as
agreed and delegated by the Health Visitor/School Nurse.
- To work with specific children and families
in the home, school or clinic setting, according to their individual care plan
as agreed with the Health Visitor, School Nurse or Team Leader. This will include children with special
health needs, in order to promote inclusion working in collaboration with other
health professionals, early years staff, school staff and parents.
- To undertake follow up of children who have
attended Accident and Emergency, as delegated by the Health Visitor/ School
Nurse. Receiving notifications, agreeing
appropriate action with the Health Visitor/School Nurse, making contact with
parents either at home or in the school and reporting back to the Health
Visitor/School Nurse. To advise on accident prevention.
- In collaboration with the School Nurse and
school education staff, provide group health promotion sessions to children
and/or their parents in areas such as play, accident prevention, dental health,
exercise, personal hygiene, healthy eating and positive parenting.
- Assisting the Integrated 0 19 Team members
in planning, organising and delivery of health promotion sessions and
displays. This may include participation
in school parents evenings.
- To have an awareness of identification of
vulnerable children who may be subject to abuse and to follow the organisations
Safeguarding policy and procedures, referring any concerns to the Health
Visitor/School Nurse.
- To support students within the work
environment to gain an understanding of the Nursery Nurses role within the Integrated
0 - 19 Service.
- To be actively involved with the ongoing
development of the Service to ensure excellent quality of care is provided to
all children, young people and their families within the borough of Tower
Hamlets.
