Job summary

As a key member of the Health Visiting Workforce Leadership team and will be responsible for the day-to-day leadership and management of a designated team of the Immunisation plus team. The post holder will achieve this by utilising best available evidence to improve the health and wellbeing of children and young people especially those from disadvantaged groups. The post holder is responsible for the day-to-day leadership the skill mix team within a defined workforce pilot area to develop and deliver universal and early intervention using an integrated model of working across professional and organisational boundaries, within the Family Hub structure. The post holder will also be responsible for the safe implementation and delivery of the enhanced pathway to enable delivery of enhanced Speech and Language screening, providing clinical expersise withing the Multi-disciplinary team and training provision across the team, Family Hubs, and within the Universal 0-19 team

Main duties of the job

  • The post Holder will be based within an identified Family Hub office in Tower Hamlets (TBC).The post holder will work to deliver the NHS EI London Immunisation Strategies and immunisations in synergy with local requirements, through Family Hubs, and working with key partners to deliver across marginalised communities The post holder will be expected to travel across Tower Hamlets as part of the day-to-day role and will work collaboratively with a wide range of external stakeholders.
  • The post holder will provide day to day operational management of the Immunisation plus Team.
  • Provide training, clinical expertise within scope of role across the workforce team, and partners as identified in relation to immunisation delivery.
  • The post holder will work as a valued member of THGPCGs and the primary care teams supporting the introduction of new immunisation programmes within agreed model, enabling and facilitating trusted relationships to be a fucus within the team delivery of key Public health interventions, , in line with national requirements.
  • The post holder will contribute to developing systems immunisation and population screening services at a local level aiming to eradicate vaccine preventable disease from London.
  • The post holder will be required to maintain constructive relationships with a broad range of stakeholders, particularly General Practitioners.

This is a fixed term role ending in March 2025.

  • The post holder will focus on the delivery of immunisation plus services whilst working in partnership with families and other health professionals/agencies, particularly GPs, on developing agreed plans and pathways to address public health function in achieving herd immunity and creating access via the Family hub programme for delivery of contact .
  • The focus of this role will be on promoting quality and clinical governance at all levels within the team through role modelling, supervision, mentorship.
  • The post holder will apply a minimum of 20% Clinical allocation either direct clinical delivery or via supervision, meetings where clinical expertise is required or supervision.
  • To be responsible for organising and prioritising own and others workload in the day-to-day allocation of work.
  • To deputise when required in the team managers absence and delegate appropriately to health care assistants.
  • To have a wide range of knowledge in approaches to communicating and managing patient care.
  • Utilise call/recall lists in partnership with local practices and ensure processes are embedded for safe delivery of vaccines.
  • Develop reports for monitoring of uptake rates of vaccination services and providing guidance of how to improve these.
