GP Care Group is delighted to announce exciting opportunities within the newly formed Waltham Forest Family Hubs Perinatal Mental Health and Parent-Infant Relationship Service. In partnership with the Family Hubs and the local authority, we are dedicated to providing a vital service for parents and babies, addressing mild to moderate perinatal and relationship difficulties. This service aims to make a lasting impact in the community and the lives of families we serve.

In addition to the roles in the above service, we are also recruiting for a Community Nursery Nurse - Infant Feeding. This role is part of the Infant Feeding Team within GP Care Group, working in collaboration with 0-19 services and HENRY to support mothers with infant feeding.

Main duties of the job

Aim of the role

The postholder will work as a member of the Infant Feeding Team within the GPCG and working incollaboration with 0-19 services and HENRY support mothers with Breastfeeding and responsive feedingand choice.

The practitioner will contact all mothers/parents /care givers between 3-5 days after birth to providesupport regarding feeding choices and required Health Promotion relating to feeding and or any issuesrelating to Health and Well-being. The CNN will escalate to the Health Visitor for further targetedassessment as required.

The CNN will offer the mother additional support via targeted contact within the home and or Family Hubsetting, or setting of parents choice .

The post holder will understand the stages of development and assist where difficulties are identified andempower families to be confident in their parenting. The post holder will work as part of the multidisciplinary team, utilising skills and knowledge to support and advise mothers across all domains of caringfor their baby.

The post holder will provide a service to women /caregivers postally as delegated by the Infant feeding Leadclinicians/Service Lead, and will participate in the delivery of interventions .

This is a community based post providing individualised assessment and care to women whose have an

existing feeding/well-being concerns or antenatal and post- natally, in line with the agreed integrated infant feeding pathway in Waltham Forest.

  • Contact mother/Caregivers 3-5 days postnatally to advise of feeding choices/support and support forwell-being.

  • Provide 121 support under delegation of named cased holder.
  • Undertake delegated Health reviews for Children .
  • Report back to the appropriate team member any significant changes in the client circumstances that may impact on safety of others.
  • To establish and maintain communications with parents, children and other professionals in a varietyof settings and mediums about daily activities, child care and child related information.

  • Provide empathy and reassurance between persons involved within scope of the role.

  • Support to families with common baby issues and signposting where needed, recognising when torefer when an issue falls outside of expected development.
  • To assess different problems to determine the course of action with a range of options to compare
  • Provision of advice and support surrounding age appropriate play and child development.

Planning and Organisational Skills:

  • Plan and manage own workload, and contribute to organisation of the team and the service
  • Work in line with the Lone Working Policy and highlight any health and safety concerns as required.
  • To be responsible for own activity data reporting.
  • Organise own time.
  • Prioritise service user care under the supervision of qualified perinatal practitioners.
  • Under the supervision of registered practitioners, making decisions about service user care asappropriate, with the support of the wider multi-disciplinary perinatal team.

Responsibility for direct/indirect patient care:

  • Work within the Local Safeguarding Board guidelines for child protection, including attendance atclient related case conferences where necessary to discuss your observations following work withthe family.
  • To access child protection supervision.
  • To maintain accurate and contemporaneous electronic records of work undertaken.
  • Ensure strict confidentiality in respect of clients and staff.
  • Provide practical and emotional support to help women prepare for the transition into parenthood.
  • Supporting partners, siblings and wider family members prior to arrival of the baby.
  • Enhancing attachment and bonding through pregnancy using current research.
  • Help to support families understand the world from the babys perspective.
  • Explore the demand and need to develop and facilitate groups to encourage socialisation andsignposting to childrens centres/Family Hubs.
