Job summary

The practitioner will hold a Specialist Community Public Health Nurse School Nurse qualification (SCPHN), with minimum of 18 months consolidated practice as SCPHN.

As an EOTAS School Nurse, you will have overall responsibility for the day-to-day management of a defined caseload of young people accessing the EOTAS programme, which covers the Pupil Referral Units/LEAP, sixth form college with PRU provision, in partnership with key partners such as Safe East, Healthspot, Mental Health in Schools teams, the Home Tuition Service, Youth Offending (Breaking the Cycle Team TH) provision.

You will hold a Clinical caseload for this cohort to support the achievement of a continuum of education provision for pupils with complex social, emotional, communication and behavioural difficulties by provision of Specialist Public Health Practitioner provision. These young people will have a variety of needs that may impact on their overall health and development.

The delivery focus will be aligned to the Health Spot model of care within Tower Hamlets. And you will work collaboratively with the NEET (Not in Education, Employment or Training) programme in supporting young people to optimise uptake and access of services that will support their management of health and well-being.

Main duties of the job

  1. To meet the health needs of a defined client group.
  2. To be the main health representative for a defined client group of young people, in assessing and meeting their identified health needs through an ongoing programme of health surveillance, intervention, monitoring, and referral.
  3. To lead on and co-ordinate community-based specialist clinics within defined area/s of practice e.g. Audiology, Enuresis, Sexual Health, Complex care management.
  4. To attend relevant panels pertaining to individual young people within the caseload or to represent a colleague/s as appropriate within the Child and Family Health Team.
  5. To be a Health lead in the facilitation of health plans for individual young people accessing the EOTAS programme. To support this process there will be positive relationships Youth Services/Home Schooled and other specialist areas to enhance social inclusion in all plans.
  6. To contribute to the curriculum planning and provision of health education programmes for these vulnerable children and young people, in the most appropriate setting.
  7. To participate in PHSE training, Youth Service Health Provision, Health access planning across YOS, Home Schooled programmes, PRUs/LEAP.

  10. Key partner in Co-Production and Co-delivery of services, engaging actively with Young people in canvassing service views for delivery and evaluation.
  11. To provide teaching and training as part of GPCG an update programme to mainstream child and family health team0-19/Partnership staff, regarding issues pertaining to this vulnerable group of young people.
  12. To be responsible for health and safety issues and to report any actual or potential hazards.

  • Registered Nurse, Children's Nurse, Midwife, or Mental Health Nurse with current NMC registration and Specialist Community Public Health Nursing qualification.
