Job summary
OneMK PCN is looking for an experienced Advanced Nurse / Clinical Practitioner to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries. OneMK PCN is delivering patient care from 3 different surgeries: Whitehouse, Neath Hill and Brooklands.
Main duties of the job
The Advanced Nurse/Clinical Practitioner will make autonomous decisions in the assessment, diagnosis and treatment of patients registered with our practices.You will have experience within a primary care setting and your high standards of patient care will naturally inspire those you work with.
In addition to standard nursing duties, the advanced nurse/clinical practitioner (ANP/ACP) role is accountable in conjunction with the lead nurse for the clinical governance of the practice and is designed to provide clinical leadership in the practice, in order to ensure the safe, effective and compliant delivery of services to all patients. You will work alongside a multi-disciplinary team including: GPs, practices nurses, wellbeing advisors, health coaches, physician associate, and more.

We are currently delivering care services in Milton Keynes from 3 different general practices: Whitehouse, Neath Hill and Brooklands, collectively known as OneMK PCN.You'll be working with a great local team and will have the support of the wider NHS services leadership team and our support centre teams as well.

Brooklands Health Centre is located in the Milton Keynes suburb of Broughton. It boasts good transport links being just a 5 minute walkaway from the nearest bus stop. The bus itself takes around 20-25 minutes from Brooklands to the City Centre. It's also well connected to the M1 being under 2 miles away from the nearest entry junction.
Whitehouse Surgery is a state-of-the-art, purpose-built Health Centre, opento all Milton Keynes residents. We are located along the V4, between the H3and H4, with ample parking.Our team offer the highest levels of care, as well as tailoring services to suitpatients health and wellbeing needs.Whitehouse Surgery is made up of GPs, Practice Nurses, Health Coaches, Pharmacists, Social Prescribers and Mental Health Workers, with further support from our PCN including First Contact Physiotherapists and Drugs/Alcohol Support in house. Our non-clinician team is made up of our practices Business Manager, Senior Administrator, and Receptionist team, with support from Medical Secretaries and Clinical Coders working in a centralised hub.
