Job summary
This is an exciting time to join HCL as a paramedic to help shape services to benefit our local population. The role is advertised as either a full time salaried post or, on a session-by-session basis,
We are looking for a paramedic with a can-do attitude who will contribute towards service development as well as see and treat patients. The main deliverables for the role will be home visiting of housebound patients and, response and triage of urgent primary care clinical issues.
Main duties of the job
The successful candidate will
· provide a rigorous and holistic assessment of patients presenting with unscheduled care needs in a variety of settings at point of first contact.
· Treat, diagnose, discharge, and refer patients safely based on assessment findings and patients own beliefs and expectations.
· Promote and develop the role of Paramedic/Paramedic Practitioner both within the Healthcare Central London GP Federation and to external stakeholders.
· Show a holistic approach to unscheduled patient care and provide assessment, treatment, and diagnosis at point of first contact by attending to patients in a variety of clinical or non-clinical settings according to patient’s needs. Refer patients to an alternative care setting or treat, review and/or discharge as appropriate.
· The Paramedic Practitioner will work within the multidisciplinary structure to provide support to member GP practices in managing the caseload of housebound patients requiring care as well as urgent visits requested by GP practices.
· Work with the service on the day-to-day triage and support of patients with acute medical issues, so be aware of plans and methods regarding delivering these services.

Provide a rapid response to urgent patient requests for home
visits and review; perform a thorough and comprehensive patient assessment and
detail a management plan to ensure that the individual needs of each patient
are met within the individual practitioner’s scope of practice
·
Effectively document clinical encounters on the electronic patient
record and effectively communicate these encounters to colleagues both within HCL
and to patients registered GP in primary care
·
Work within the multidisciplinary structure of HCL, supporting
colleagues within the team to help provide healthcare services within patients’
own home whenever possible to prevent non elective admission into the acute
hospital
·
Organise and when available undertake diagnostic tests as
required, for example taking blood and other samples for analysis
·
Independently assess needs for, and perform therapeutic procedures
such as joint immobilisation, pharmacological intervention in the form of
patient group directions for oral medications or injections, immunisations, and
wound management
·
Liaise with other agencies/disciplines (primarily general
practice) involved in the provision of care and treatment of patients and
promote and develop cross partnership working
·
Maintain required standards of clinical competence and
qualifications through continuing professional development
·
Demonstrate an understanding and application of evidence-based
practice
·
Where necessary, ensure continuity of care by following
up on and managing patients already seen by another
health care professional
·
Demonstrate an ability to process and communicate complex facts,
and interpret and act appropriately on the information with a large degree of
autonomy
·
Demonstrate a high standard of clinical information collection and
documentation, and a use of all guidance around the collection and storage of
information under the existing local and national guidance

- Minimum of 5 years post registration qualification as a Paramedic or equivalent relevant experience in front line ambulance or acute or chronic care
- Proven previous experience of working within the NHS, in a multidisciplinary team in a similar role
- Experience of using databases, Word, and Excel
