Job summary

Knaresborough and Rural Primary Care Network have an exciting opportunity to recruit an Advanced Nurse Practitioner to join their friendly teams. The role involves working closely with the primary care teams of 3 GP practices in this 6-practice PCN.

We are looking for an experienced Advanced Nurse Practitioner to work autonomously in practice and the community, responding to urgent and same day requests for acute and long term conditions. You will be confident in assessing, triaging, diagnosing and treating acute presentations, minor illness and injuries, making judgements and managing varying degrees of risk in complex cases. This includes prescribing or issuing medication, performing basic investigations and supporting our care homes.

Main duties of the job

You will work with the practices to provide a specialist Advanced Nurse Practitioner resource, working in collaboration with other members of the Multidisciplinary Team.

You will be an autonomous, accountable Advanced Nurse Practitioner, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment, to deliver quality patient services.

Our PCN covers a large, and beautiful geographical area stretching from the wind swept vistas of Nidderdale to the historic centre of Knaresborough, providing a magnificent back drop to home visits.

Job responsibilities

  • Assess patients' health status, triage and diagnose illnesses, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE national and local clinical guidelines and local care pathways).
  • Manage and coordinate care for patients with chronic conditions, ensuring continuity of care and optimising health outcomes.
  • As an independent prescriber, prescribe safe, effective, and appropriate medications as defined by current legislation. Monitor, and adjust medications to address acute and chronic health issues, promoting medication adherence and safety.
  • Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
  • Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
  • Offer a holistic service to patients and their families, developing personalised care plans.
  • Manage undifferentiated, undiagnosed condition and identify red flags and underlying serious pathology, taking appropriate action.
  • Educate patients and caregivers on disease management, self-care, and preventive measures to enhance patient empowerment and autonomy.
  • Respond to urgent situations and provide acute care interventions, ensuring patients receive appropriate medical attention.
  • Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.
  • Refer patients directly to other services/agencies as appropriate.
  • Work with stakeholders including the GP practices in the Primary Care Network and the wider ICB to promote integrated and seamless pathways of care.

Qualifications

Essential

  • Registered first level nurse.
  • Have either graduated from a Centre for Advancing Practice accredited MSc advanced practice program or completed the Centres ePortfolio (supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre.
  • Relevant clinical/health degree.
  • Non-Medical prescribing.

Desirable

  • Community nursing specialist qualification.
  • Mentor/teaching qualification.

Experience

Essential

  • Significant demonstrable post-registration experience.
  • Recent Primary Care and/or unplanned and urgent care experience.
