Job summary

Heart of Harrogate Primary Care Network (HoH PCN) is a four Practice network based across the centre of Harrogate Town with over 50,000 patients. The four Practices have 27 aligned Care Homes and a growing elderly patient population which has a significant impact on Practice workload. The aim of the HoH PCN Care Home Team is to improve and streamline the support provided by Primary Care. Working as a specialist single multi-disciplinary Care Home Team, overseen by a Doctor Lead, they will tackle health inequalities using a proactive, preventative approach.

Interviews will be held on Tuesday 19th December 2023.

Main duties of the job

This role is pivotal in ensuring all Care Home patients receive the best possible care and service. The post holder will handle incoming and outgoing communications, analyse data and provide administrative support to the Care Home Team to ensure that care home patient contacts are triaged and coordinated, working to standard operating procedures.

Key Responsibilities

The Care Coordinator will:

  • Be the Care Home Team first point of contact either by phone or email as required for care home staff, carers, families and Practices.
  • Triage patient contacts according to a standard operating procedure to ensure they are handled appropriately by the right team member.
  • Organise care home visits for Care Home Team clinical colleagues.
  • Ensure patient records and other data is maintained and updated in SystmOne and other software packages.
  • Support the GP Lead and Project Manager in preparation of reports for KPIs including data entry and extraction.
  • Data cleansing and updating of SystmOne, Ardens and other software as required.
  • Work closely with Practice/s and PCN teams. The Care Coordinator will have a named contact in each practice to support the role.
  • Maintain regular contact with each of the 4 PCN practices Nominated Data Quality Leads to ensure accurate coding and records are maintained.
  • Maintain all statutory and mandatory training including information governance principles with up-to-date knowledge of basic safeguarding processes in place for vulnerable individuals.

  • Experience of working in healthcare.
  • Experience of working with patient information systems e.g. SYSTMONE, ACCURX and similar.
  • Experience of working with Word and Excel.
  • Experience in a patient or customer service facing role.

Desirable

  • Experience of line management/supervision or coordination of staff.
  • Experience of coding in a primary care setting.
  • Meeting coordination, including diary management.
  • Experience of working as part of multi-disciplinary team.

Qualifications

  • Educated to GCSE level or equivalent including Mathematics and English (C or above).
  • Willing to undertake specialist training provided by the Personalised Care Institute.

Desirable

  • NVQ level 3 or A levels in relevant disciplines.
