Job summary
The ProActive Care Team works closely with 17 GP practices in
the YHN umbrella to provide preventative and a pro active element
of care. We support the practices to identify the patients that would most
benefit from pro active intervention, undertake specialist OT assessments and
intervention in the patients home and signpost to appropriate support services,
if needed. The team also undertake Advanced Care planning linking with practice
and wider community MDT. We are a dynamic forward thinking team, welcoming

The
Occupational Therapist will focus on assessment, pro-active care management,
ensuring that patients are identified and managed supporting patients to have
the best quality of care through pro-active planning.
We would consider less than full time working if required but would require a minimum of 0.6
Main duties of the job
You will be a highly motivated qualified
Occupational Therapist with a passion for providing excellent patient care,
this is a fantastic opportunity to be part of a new programme, alongside a team
of colleagues providing pro active care to support patients to have quality of
life and appropriate care at the point of need. You will have experience of
working in a healthcare environment, including physical and mental health
settings, be able to communicate with patients, carers and families and able to
function effectively as part of a multi-disciplinary team. You will be familiar
with IT tools and well versed in using SystmOne across all practices and various
embedded templates and reporting tools.
About us
This
is an exciting developing project with all 4 of the PCNs within the Yorkshire
Health Network working together to achieve the aims of the project. The role
will be mobile and cover the whole geography drawing patients from our practices
who range from Masham, through Ripon, central and outskirts of Harrogate, Knaresborough,
through to Pateley Bridge, Green Hammerton and Boroughbridge.
As PCNs

Clinical
supervision is provided to the Occupational Therapists by the Lead Occupational
Therapist and we have regular PACT peer support and team meetings.

is a pilot project for the Pro-Active Care Team (PACT) working across 4 Primary Care Networks who all work
closely together covering the Harrogate and Rural area. The PACT model helps to achieve the targets
of the new PCN enhanced service which includes Tackling neighbourhood
inequalities, anticipatory care and offering a personalised care to a
vulnerable group in our community.
We expect the majority of these
patients to be Housebound and frail who have unmet health and social
needs. The PACT team will visit these
patients in the community and support to increase Quality life years.
The main aim of the team is to work with housebound
and frail elderly patients to pro-actively identify care needs before the
patient is in crisis, helping to prevent admissions, improve independence and
ensure patients wishes are taken into consideration at all times . The team
will focus on identifying those patients on lists for all of the practices
within YHN and as part of a team visit the patient, make assessments, signpost
to the appropriate support service, if needed.
