Job summary

Knaresborough and Rural PCN have an exciting opportunity to recruit a Paramedic to join their friendly team. The role involves working closely with the primary care teams of the 3 GP practices based within the centre of Knaresborough.

We are looking for an experienced band 7 practitioner to work autonomously in practice and the community, responding to urgent and same day requests for acute and long term conditions. You will be confident in assessing, triaging, diagnosing and treating acute presentations, minor illness and injuries, making judgements and managing varying degrees of risk in complex cases. This includes prescribing or issuing medication, performing basic investigations and supporting our care homes.

Main duties of the job

You will work with the practices to provide a specialist Advanced Paramedic resource, working in collaboration with other members of the Multidisciplinary Team.

You will be an autonomous, accountable Advanced Paramedic Practitioner, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment, to deliver quality patient services.

You will also be taking part in activities within the surgery including telephone calls, and minor illness clinics.

Supporting and educating service users/patients, promoting self-care and empower them to make informed choices about their treatment.

Key Responsibilities

Working as part of the multi disciplinary team with the PCN and wider community health and social care partners

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team, either as part of a home visiting service or same day care within practices

Triage and treat patients, making any necessary referrals to other members of the primary health care team or organise admission if required.

Undertake the following if required:

o perform specialist health checks and reviews within scope of practice and in line with local and national guidance

o perform and interpret ECGs

o perform investigatory procedures as required, and

o undertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within scope of practice, and within line of local and national guidance.

Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations

Advise patients on maintaining health and refer to allied services eg Social Prescriber Link Worker when a health promotion need is identified.

Support the delivery of anticipatory care plans and lead certain services (e.g. monitoring blood pressure and diabetes risk of elderly patients).

Communicate at all levels within the multidisciplinary team ensuring an effective person centred service is delivered attending and contributing to meetings as required

Maintain accurate, contemporaneous healthcare records appropriate to the consultation

Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high-quality patient care

Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices.

Work within local policies and procedure

Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service

Achieve and demonstrate agreed standards of personal and professional development in order to meet the needs of the service

Participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need

Actively participate in mentoring and supervision, plus practice-derived CPD opportunities

Contribute positively to the effectiveness and efficiency of the team and work colleagues

Take responsibility for safeguarding children and vulnerable adults and work with other team members to ensure they are protected

Work closely with the multi-professional, management and administration teams to ensure the smooth running of the clinics/PCN home visiting service, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the team

Work closely with the Enhanced health and care home team and wider PCN and Community team.

It will be necessary to attend and contribute to various PCN and MDT meetings as requested. The only reason for not attending will be annual, study or sick leave or if a non-working day

develop and agree a personal development plan (PDP) utilising a reflective approach to practice, operating under appropriate clinical supervision

  • Undertake statutory and mandatory training as required by the Practice/s.
  • Demonstrate clinical leadership.
  • Undertake appropriate clinical and non clinical CPD and record the same
  • If not already at Level 7 capability in paramedic areas of practice or equivalent (such as advanced assessment diagnosis and treatment) the post holder must participate in clinical supervision with Yorkshire Ambulance Service.
  • Pro-actively promote the role of the Paramedic within the PCN and externally to key stakeholders and agencies.
  • Respect patient confidentiality at all times and not divulge patient information unless sanctioned by the requirements of the role

  • Registered with the Health and Care Professions Council (HCPC)
  • completed their two-year Consolidation of Learning period as a newly qualified paramedic
