Knaresborough and Rural PCN is looking for an
motivated and aspirational clinical pharmacist to join our expanding Pharmacy
Team. This is an exciting opportunity to be part of our well established and
experienced pharmacy team working closely with GPs and AHPs across the PCN and
the wider locality .
The role of the clinical pharmacist is changing
rapidly. Primary care needs to be ambitious in pro-actively helping patients
stay safe and well and out of hospital as well as ensuring that general
practice remains resilient in their ability to provide the increasing range of
services. This role is critical in supporting the delivery of the new Primary
Care Network Contract DES specifications and will help lead improvements to
maximise safe, cost effective best practice to improve the quality of patient
care
Main duties of the job
You will work as part of a multi-disciplinary team
in a patient facing role, reporting to our Senior Clinical Pharmacist . You
will be taking responsibility for chronic disease management, medication
reviews to proactively manage patients with complex conditions and
polypharmacy, improving medicine optimisation and safety, supporting care
homes, as well as practice clinics.
You will require flexibility to develop in line
with the National service specification in the GP contract and Long Term Plan
and in respond to the evolving needs of PCNs
You will be a forward-thinking, flexible,
innovative and enthusiastic about working collaboratively across the team and
keen to deliver the ethos of the network.
If you have not already completed or commenced the
CPPE pathway you will be enrolled on the CPPE PCN pathway to support this role.
and may be expected to undertake the Independent Prescribing qualification.
Both these learning opportunities would be fully supported.

About us
Knaresborough and Rural District PCN consists of 6 practice
with diverse population from suburban to rural, dispensing and
non-dispensing. Together they provide care for around 50,000 patients.
All of the Practices within the PCN’s operate
multi-disciplinary teams with GP’s Nursing, Social Prescribing Link Workers,
First Contact Physiotherapists. Paramedics and Pharmacy teams. The teams are
expanding and diversifying to support the patient population and identified
needs within that, the patient experience is at the heart of everything for our
PCN’s.
The post holders will benefit from to
professional supervision, a nominated clinical supervisor and a strong support
and peer network from the established team. All of the practices have had
pharmacists working within them alongside the Pharmacy Technicians. The team
already has excellent links with the local hospital, community pharmacies, care
homes and other practice pharmacists in the area, so you would be truly part of
a big team working for better patient care

The post holder will work within their clinical
competencies with ongoing support and supervision from the PCN Clinical
Pharmacist as part of a multi-disciplinary team. The role will provide
expertise in clinical medicines management, provide face to face structured
medication reviews, manage long term conditions, management of medicines on
transfer of care and systems for safer prescribing, manage repeat prescription
authorisations and reauthorisation, and acute prescription requests for
patients

supported ongoing development.
Work as part
of a multi-disciplinary team in a patient facing role to clinically assess and
treat patients using their expert knowledge of medicines for specific disease
areas
Take
responsibility for the care management of patients with chronic diseases and
undertake clinical medication reviews to proactively manage people with complex
polypharmacy, especially the elderly, people in care homes, those with multiple
co-morbidities (in particular frailty, COPD and asthma) and people with
learning disabilities or autism (through STOMP Stop Over Medication
Programme);
Be a
prescriber, or be willing to undertake training to become a prescriber, and
work with and alongside the general practice team
Provide
specialist expertise in the use of medicines whilst helping to address both the
public health and social care needs of patients at the PCNs practice(s) and to
help in tackling inequalities
Provide
leadership on person-centred medicines optimisation (including ensuring
prescribers in the practice conserve antibiotics in line with local
antimicrobial stewardship guidance) and quality improvement, whilst
contributing to the quality and outcomes framework and enhanced services
Through
structured medication reviews, support patients to take their medications to
get the best from them, reduce waste and promote self care
Have a
leadership role in supporting further integration of general practice with the
wider healthcare teams (including community and hospital pharmacy) to help
improve patient outcomes, ensure better access to healthcare and help manage
general practice workload
Develop
relationships and work closely with other pharmacy professionals across PCNs
and the wider health and social care system
Take a
central role in the clinical aspects of shared care protocols, clinical
research with medicines, liaison with specialist pharmacists (including mental
health and reduction of inappropriate antipsychotic use in people with learning
difficulties), liaison with community pharmacists and anticoagulation
To provide
support to pharmacist colleagues in the clinical aspects of medicine reviews,
clinical audit, practice research and governance
Share
outcomes of prescribing, clinical, service and patient outcomes audits within
the PCN and across the locality in order
to drive service and practice improvement
To respond to
and act on MHRA drug alerts, implementing and communicating any medication
changes required as a result
To monitor

- Mandatory registration with the General Pharmaceutical Council
- Vocational Masters degree in Pharmacy (MPharm) or equivalent Commitment to become an independent prescriber
Desirable

- Specialist knowledge through a postgraduate diploma or equivalent
