Job summary

Yorkshire Health Network (YHN) are looking to recruit entry level Clinical Pharmacists to work as part of a multi-disciplinary team within our Primary Care Networks (PCNs). This role will work alongside our Clinical Pharmacists and with support and supervision will progress to full Clinical Pharmacist role within 12-18 months

The successful candidates will have support the Clinical Pharmacists with patient facing roles in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care and will have a supervised caseload.

Main duties of the job

Successful candidates will work as part of a multi-disciplinary team to develop and run processes for repeat prescription re-authorisation, management of medicines on transfer of care and systems for safer prescribing.

Successful candidates will have excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews.

in return, YHN will provide support to develop the role; to include clinical supervision, mentorship and dedicated study time to undertake the CPPE primary care pharmacy education pathway and an Independent Prescriber qualification.

Experience in General Practice would be an advantage but is not essential if candidates are clinically up-to-date and understand the process of a full medication review in the primary care setting.

Our PCN Junior Clinical Pharmacists will benefit from peer support from other pharmacists within YHN, with shared line management and supervision facilitated and supported by us.

Job responsibilities

Job purpose

The post holder will work within their clinical competencies with ongoing support and supervision from the PCN Clinical Pharmacist as part of a multi-disciplinary team. The role will provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, and acute prescription requests for patients in GP practices within one of the four Primary Care Networks in Harrogate and Rural District.

the PCN Clinical Pharmacist (Entry-level) role will progress to that of the full remit of Clinical Pharmacist after the successful completion of key competencies and attainment of agreed objectives within 12 months of commencement. Your clinical supervisor and wider pharmacy team will assist in the evaluation of the competencies throughout the 12 months and progression will be automatic on attainment. This may be extended to 18 months in exceptional circumstances.

Key Responsibilities

The following describes the key responsibilities that the postholder will deliver and/or work towards delivering after supported ongoing development.

Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

Under the supervision of the PCN Clinical Pharmacist undertake care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme).

Be a prescriber, or be willing to undertake training to become a prescriber, and work with and alongside the general practice team

Alongside the PCN Clinical Pharmacist to provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice(s) and to help in tackling inequalities

Provide support to the Clinical Pharmacist with person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services

Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care

Take a designated role in supporting the further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload

Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system

Provide support to the PCN Clinical Pharmacist in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists and anticoagulation

Be part of a professional clinical network and have access to appropriate clinical supervision.

Qualifications

Essential

  • Mandatory registration with the General Pharmaceutical Council
  • Vocational Masters degree in Pharmacy (MPharm) or equivalent
  • Commitment to become an independent prescriber

Desirable

  • Membership Primary Care Pharmacy Association (PCPA)
  • Membership of the Royal Pharmaceutical Society
  • Specialist knowledge through a postgraduate diploma or equivalent
  • Independent prescribing qualification

Experience

Essential

  • At least 1 years post registration experience in a hospital, community or general practice setting
  • Capacity to be innovative and to develop the role
  • In depth therapeutic and clinical knowledge and understanding of principles of evidence based healthcare
  • Excellent interpersonal, influencing and negotiating skills
  • Excellent interpersonal, communication (written and oral), presentation and influencing skills
  • Ability to plan, manage, monitor, advice and review general medicine optimisation issues in core areas for long term conditions.
  • Competent user of Microsoft Office programmes, including Outlook, Excel and Word to create simple plans and reports
