Job summary

We are seeking an enthusiastic and highly motivated Pharmacy Technician to join our friendly and high achieving team for the Heart of Harrogate PCN (Primary Care Network) at Kingswood Surgery, based in Harrogate.

This is a unique part time opportunity to work 22 hours per week. We offer flexibility for hybrid work arrangements. However, please note that regular on-site presence is essential to maintain the high standard of patient care and effective teamwork required in our GP practices.

This is an exciting opportunity for the successful candidate to provide excellent care alongside our wider clinical team which includes GPs, ACPs, Nurses, Nurse Associates, Health Care Assistants, First Contact Physiotherapists, First Contact Mental Health Practitioners, Social Prescribers, and our Care Navigator Team.

Main duties of the job

The role will support Kingswood Surgery to ensure the delivery of safe, effective, and efficient systems for medicines and related care including repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.

Heart of Harrogate PCN is comprised of 4 practices, Dr Moss and Partners, Kingswood, Leeds Road Practice and Church Avenue all of whom are situated within the boundaries of the beautiful spa town of Harrogate.

This role is employed in partnership with Yorkshire Health Network (YHN).YHN is the local federation for all 17 GP Practices across Harrogate and Rural areas, made up of fours PCNs. YHN and the PCNs work collaboratively to deliver a supportive structure which is focused on achieving the best results from a joint management and clinical delivery setup for the benefits of patients across the region.

Job description

Job responsibilities

The following describes the key responsibilities that the postholder will deliver and/or work towards delivering after supported ongoing development.

  • Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.
  • Carry out medicine optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation.
  • Utilise a range of consultation and communication skills to work in partnership with patients to ensure they use their medicines effectively.
  • Support, as determined by the PCN, medication reviews and medicines reconciliation for new care home patients, synchronising medicines for patient transfers between care settings and linking with local community pharmacies.
  • Provide specialist expertise, where competent, to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.
  • Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. This will be achieved through undertaking clinical audits (e.g., use of antibiotics), supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services.
  • Work with the PCN multi-disciplinary team to ensure efficient medicines optimisation.
  • Triage general medication requests allowing GPs and clinical pharmacists to review those that are more complex.
  • Develop relationships with other pharmacy technicians, pharmacists, and members of the multi-disciplinary team to support integration of the pharmacy team across health and social care including primary care, community pharmacy, secondary care, and mental health.

  • Mandatory registration with the General Pharmaceutical Council.
  • NVQ Level 3 in Pharmaceutical Services.

Experience

  • Previous work experience in a pharmacy or healthcare setting.
  • Competent user of Microsoft Office programmes, including Outlook, Excel and Word to create simple plans and reports.
  • Ability to maintain effective working relationships and promote collaborative working.

Desirable

  • Previous work experience in a primary care setting or GP practice.
  • Experience with using SystmOne clinical system.
