Job summary

Tri Locality Care Ltd, a GP Federation, holds fixed-term NHS contracts with Hampshire & IOW ICB and 3 PCNs to provide a number of primary care services in the community, such as complex wound care and a Transformation team tasked with preventing hospital admissions for frail/elderly patients.

There are opportunities to join the above teams and to be considered for Lead roles such as TLC lead for infection prevention control across all services. Thiswill be in addition to the tasks identified in the job description and will be discussed with the successful candidate.

The Complex Wound Nurse is a specialised healthcare professional responsible for providing expert care and management of complex wound conditions in a primary care setting. This role involves assessing, planning, implementing, and evaluating wound care for patients with chronic and acute wounds. The primary focus is on promoting wound healing, preventing complications, and improving the overall quality of life for patients with complex wounds.

TLC holds an annual contract to provide this service to named general practices from the Eastleigh area. The service is currently provided at Knightwood Surgery premises, Chandlers Ford.

The Transformation Team provide holistic assessments for frail and/or elderly patients to prevent hospital admissions, undertaking home visits and signposting to other appropriate services, including voluntary sector organisations.

Main duties of the job

1. Wound Assessment and Diagnosis: Conduct thorough assessments of patients with complex wounds, considering factors such as wound size, depth, and characteristic and plan treatment:Develop individualised and evidence-based treatment plans for patients with complex wounds, taking into account the patient's overall health status, co-morbidities, and lifestyle

3. Wound Care Intervention and Patient Education Perform advanced wound care interventions, including debridement, dressing changes, and application of specialised wound care products.Educate patients and their families on proper wound care techniques, self-management strategies, and lifestyle modifications to support optimal wound healing

5. Documentation, Reporting and improvement:Maintain accurate and detailed electronic health records (EHR) documenting the assessment, interventions, and outcomes of wound care.Participate in quality improvement initiatives to enhance the overall quality of wound care services provided in the primary care setting.

7. Collaboration and Communication:Collaborate with primary healthcare professionals, specialists, and other healthcare providers to ensure seamless and coordinated care for patients with complex wounds

8. Compliance and Regulatory Adherence:Adhere to all relevant healthcare regulations, standards, and policies related to wound care, including infection prevention policies.

About us

TLC Ltd has existed as a GP Federation since 2016. Whilst many of our NHS contracts have been fixed term for one to two years, we are still here and still providing primary medical services on behalf of our GP shareholders. Staff have been employed on fixed-term employment contracts to mirror the service contracts, but many staff employed in 2016 are still employees with us today.

The Transformation Team was introduced in 2017 and provides a very highly valued service by both patients and GP's. A rare chance to join this multi skilled team.

The complex wound care team welcome the opportunity for an additional Nurse to join them. Clinics are generally held on Tuesdays and Fridays with occasional varying of the days during Bank Holiday weeks etc. Practice Nurses with wound care training and practice nurse experience would be ideal candidates for this vacancy.

There may also be an opportunity to take cervical cytology smears, assist at coil clinics etc for a suitably qualified and experienced primary care nurse. These services would be provided during enhanced access services which are evenings and Saturdays.

Generate reports and communicate effectively with other healthcare professionals regarding the patient's progress and any necessary adjustments to the treatment plan.

6. Quality Improvement:

Participate in quality improvement initiatives to enhance the overall quality of wound care services provided in the primary care setting.

Stay updated on the latest evidence-based practices and contribute to the development of best practices for wound care.

7. Collaboration and Communication:
