Job summary

We are looking for committed, motivated and resilient Support Workers who are passionate about supporting young people aged 16 to 25, who are transitioning from residential or foster care and may have some struggles with their mental health.

You will be required to work as part of a team to deliver guidance and support to young people within our service. You will be required to communicate with our Occupational Therapist and management team to ensure the needs of the YP are met.

Main duties of the job

As a Support Worker your role will be to work alongside young people, actively promoting independence and preparing them to transition into adulthood.

Working on a multiagency basis and in partnership with internal and external professionals to ensure young people are given the correct wrap around support and are treated equally, as individuals, and without discrimination.

If you have excellent written and verbal communication skills, are resilient and committed to the support worker role and most importantly you will conduct yourself as a positive role model for young people and colleagues we want to hear from you.

About us

Associated Wellbeing is a thriving and growing company supporting young people across the country who struggle with their mental health needs. The company has a tier 3 inpatient unit based in Darwen, Lancashire which supports young people within a theraputic 12 week assessment unit. We also have a 12 bed placement for young people aged 16+ on Leeds Road in Bradford. We have a fantastic Outreach programme which supports community involvement and independent living, our company is also in the registration process of opening up our first OFSTED Children's home in Burnley. Job responsibilities

To provide a caring, supportive, and nurturing environment in which children can thrive, achieve, feel safe, secure, and live free from harm

To ensure and promote the safe care and safeguarding of all children

To work in line with Associated Wellbeings ethos and values, best practices, and National Care Standards

To work with young people to assist them to understand previous life experiences, responding appropriately to ensure mental health needs are met

To provide advice, assistance, and support on a 1:1 basis to enable children to have the opportunity to address past and present difficulties

To provide appropriate personal care and a safe, clean environment to children and ensure their physical comfort, health, and wellbeing, and support them through positive living experiences

To assist in the assessment, planning and delivery of services to support the care and support needs of children using the service

Contribute to the development and review of individual support plans and work to these plans on a day-to-day basis

Using yourself through developed relationships, to support and work with children living within the home to ensure their needs are responded to within a therapeutic environment

To actively contribute to the social and academic education of the children

To understand, work to, and constructively contribute to the ongoing review and development of the homes policies, procedures, practices, and processes

To contribute to recording systems designed to meet the care needs of children

To provide outreach work or throughcare guidance or aftercare support to children in line with agreed plans

To work in collaboration with other aspects of the organisation and external professionals in the best interests of children

Where appropriate, to work directly with families / carers

To engage positively with children when managing behaviour using management behaviour techniques and implementing appropriate strategies

To manage recording systems for finance and administration in line with policy and procedure

To contribute to the daily routines of the home and participation in household tasks (cooking, cleaning, general maintenance around the home)

To encourage children to accept shared responsibility for the upkeep of their home, and in doing so make a valuable contribution to its running so they can develop independent living skills and experience family living

To empower children and facilitate their active involvement in any decision-making about their lives and future

To encourage and support children to make links within the community, and to expand their social network

To attend and contribute to weekly house meetings with children

To be able to work with groups of children in a domestic setting to ensure that their needs are met collectively and individually
