Phoenix Health PCN provides a range of services to the patients, communities and practices within the PCN area which is based at our offices in Chinnor and covers the practice area of our 3 member practices: Cross Keys Practice, Haddenham Medical Centre and Unity Health.

Main duties of the job

We are looking for a compassionate, collaborative and motivated coordinator to support the delivery of care to vulnerable patients and care homes, coordinating the work of healthcare professionals and non-clinical staff involved in the care of patients.

The successful candidate will possess strong administrative skills with the ability to multi task and ability to communicate well with the patients and wider external agencies. Often being the first point of contact for our patients must be able to communicate and listen to patients clearly and effectively, demonstrate empathy putting our patients at ease.

The Team Co-ordinator will provide support to the healthcare professionals who deliver our services including the PCN Team and third party providers.

Key Responsibilites

1. Ensure the PCN Hub appointment book is set up for the paramedics, social prescribers, health and well-being coaches and third party providers who deliver in-hours and enhanced access for the PCN.

2. Act as the main contact point for changes to the appointment book, communicating any alterations in a timely manner to practices.

3. Ensuring an adequate stock of equipment and office supplies are available for PCN staff, overseeing an ordering system and keep stock records.

4. Providing admin support to social prescribers and health and well-being coaches as agreed with the Operational Manager. This may include providing admin support to group consultations and ensuring a supply of up to date promotional materials are available for use by the different roles.

5. Check discharge notifications and take appropriate action. This may include contacting patients/carers to signpost to support or referring on to paramedics or social prescribers.

6. Collect, analyse and report on service delivery using Emis searches.

7. Work collaboratively with internal PCN colleagues particularly the Pharmacy Team Co-ordinator, Social Prescribers and Health and Well-being Coaches to build strong working relationships.

8. Provide administrative support to weekly Team Meetings.

9. Work as part of the PCN team to promote a positive image of the PCN and ensure its business continuity.
