Job summary

To support and assist qualified staff in the assessment, planning and implementation of patient care. Delivering care to the patient as prescribed by the individual care plans. To assist in the provision of a high quality service that enables the patients to develop an independent and individual lifestyle and to work in accordance with the stated support plans, policies, philosophy and aims and objectives of Maple House. Patients packages of care will mirror, local authority assessments and allocate shared and one to one time for specific tasks, which are documented in individual care plans. Staff will be required also to provide effective communication within the immediate team and to the management team. Such communication will include daily issues and contribution towards all parts of the service such as reviews for patients, ward meetings, staff meetings, health and safety issues etc.

Job responsibilities

SPECIFIC RESPONSIBILITIES:

To work a pattern of shifts and ensure that the patients who they are working with are supported in line with their support plan and receive consistent support.

To assist qualified staff with the assessment and implementation of individual patient care plans undertaking routine tasks and activities as directed to facilitate the well-being, dignity and treatment of all patients.

To provide practical support and responsive care to patients who require assistance with intimate personal needs such as dressing, bathing and toileting activities ensuring that such activities comply with procedures and the relevant healthcare legislation.

To observe and monitor the well-being of patients ensuring that any unusual physical, mental or emotional occurrences are promptly referred to senior staff or the nurse and documented as appropriate.

To develop and maintain a good professional relationship with patients, colleagues and visitors responding promptly and courteously to requests and enquiries.

To follow any lists and plans in place on their ward such as daily checks, task and delegation sheets as highlighted at induction stage.

To liaise when necessary with the management team, in regard to patient issues, staffing issues, shift planning, meeting needs and proving effective communication when handing over to other support staff or management.

To oversee and maintain records kept within the patients file and to ensure there is enough blank paperwork in order to complete said tasks.

To support patients with any health related support issues where applicable, e.g. making and attending Dr.s appointments, and advocating where the need is assessed.

To support patients with day to day support tasks as highlighted in their support plans.

To work with a philosophy of joint working with patients in the delivery of needs led services, to enable them to increase independence in agreed areas of their lives.

To deliver care and support within the regulatory requirements of C.Q.C.

To liaise with patients and other support staff to run diaries and to coordinate appointments / activities at times which are convenient to patients.

To participate and contribute towards the process of review, inclusive of reviews of support plans, risk assessments and annual reviews, where assigned.

To follow through any additional tasks which have been delegated.

To help patients cook healthily and maintain a clean and safe bedroom.

To make the management aware of any concerns, incidents or events that may affect the safety of a patient or staff. (Whilst acting within the organisations confidentiality policy).

To undertake mandatory and other training identified as a requirement by the organisation.

To comply with company policies and procedures, the Data Protection Act 1998 and work in accordance with the Management of Health and Safety at Work Regulations 1999 and Health and Safety at Work Act 1974.

The post holder may be expected to undertake other reasonable duties as required by the COO, Governance Director, Registered Manager, Deputy Manager, Lead Nurse or Head of Clinical Services.
