General Practitioner - SAS Service

Job summary

We are seeking a dedicated and experienced General Practitioner to lead our Special Allocations Scheme (SAS) a vital service providing care to patients removed from mainstream GP surgeries due to challenging behaviours.

This role offers a unique opportunity to deliver compassionate, holistic care to complex patients in a supportive, multidisciplinary environment. The position includes a mix of remote triage and face-to-face consultations, with flexibility to suit your working preferences.

Must be available Monday or Thursday for daytime hours

Main duties of the job

Key Responsibilities

  • Deliver first-contact care for patients with complex physical and mental health needs
  • Perform remote triage and in-person consultations
  • Develop and manage patient-centred care plans
  • Prescribe and refer appropriately in line with current best practices
  • Maintain accurate clinical records and adhere to GDPR guidelines
  • Coordinate with other healthcare professionals and attend MDT/Best Interest meetings
  • Participate in audits and clinical governance activities
  • Support practice administration and attend practice meetings

Essential Requirements

  • MRCGP and on the NHS Performers List (with no restrictions)
  • Experience working in primary care settings
  • Excellent communication and organisational skills
  • Commitment to ongoing professional development
  • Enhanced DBS clearance
  • Proficient in clinical systems such as Adastra, EMIS, SystmOne, EPS
  • Must have the right to work in the UK

Patient Care: provide first-contact care for patients with undifferentiated physical and psychological health problems, using their skills in history-taking, physical examination, and clinical decision-making to establish diagnoses and management plans. This includes provision of care for emergency conditions (and appropriate management) and ensuring chronic health needs are met.

Continuity of Care: ensuring continuity of care by maintaining ongoing relationships with patients, managing chronic conditions, and coordinating care with other healthcare providers.

Preventive Care: provide preventive care, including health education, vaccinations, and screening programs to prevent illness and promote health.

Best practice : ensuring appropriate prescribing individualised to the patient within the scope of primary care. Working in line with current up to date guidelines. Requesting appropriate investigations and following up on results as they are received
