Job summary

PHL Group have an exciting opportunity for experiencedAdvanced Clinical Practitionersto join us in both our Out of Hours service and triage.

As an Advanced Clinical Practitioner, you will be accountable for delivering safe patient care to a high standard. You will need to be confident in working independently and autonomously at an advanced level of practice across our services, including remote telephone triage, and face-to-face consultations in a primary or urgent care clinic. Acting within your professional boundaries, your duties will include all aspects of the patient journey including initial history taking, clinical assessment, diagnosis and treatment, prescribing medication, and evaluation of the patients care. Advanced practice will include evidence of working within the four pillars: clinical, education, research and leadership.

Main duties of the job

  • Prescribe medication as required within your clinical competence.
  • Recognise the need for and initiate emergency treatment to save a life or to safeguard the interests of the patient and refer to emergency services when appropriate.
  • Request investigations and be able to interpret pathology and radiology results to ensure appropriate treatment and/or onward referrals.

  • 3 years of experience working in a primary care GP practice.
  • 3 years experience in an urgent care setting such as an urgent treatment centre or Emergency Department.
  • At least 6 years post qualification.
  • Excellent knowledge and experience in the assessment and management of minor injury and illness presentations with evidence of competency completion for role.
  • Experience of working autonomously at level that demonstrates the four pillars of advanced practice.
  • Experience of working in a multi-disciplinary team including mentoring of junior staff.
  • Good knowledge of primary care referral pathways such as 2-week-wait referrals.
