This one role would suit someone who is open to exploring a portfolio role within PHL, with the opportunity to work within a whole range of services.

Hours:

5-6 sessions per week, Mon-Fri, split across PHL services. Salary negotiable depending on experience.

Main duties of the job

As part of a wide range of services we deliver, PHL are the contract holders for the Hampshire wide (excluding Southampton) Special Allocations Scheme (SAS), which provides Primary Care Services to patients who have been deregistered from their originating GP Practice. These patients have their care provided by a dedicated clinical and operational team and we are looking to recruit additional GP support for the service, in what can be either a structured or flexible arrangement, working closely as part of this fantastic team (number of weekly hours/sessions open to further discussion). The post holder will primarily provide clinical assessment to our registered patient population (approx. 125 patients) via both telephone and face-to-face consultations. They will work closely with the SAS team and the CCG to develop and grow this unique service, building close links and relationships with secondary care and GP service colleagues and those working in the wider healthcare community. The post holder will ensure that patients receive optimal care in the most appropriate setting, whilst delivering the objectives of the service and delivering Enhanced Services, QOF, CQUINS and complying with CQC and other statutory bodies requirements.

About us

Partnering Health Group now run PHL Integrated Urgent Care (formerly known as Hampshire Doctors on Call). The former Out of Hours model of care has evolved into Integrated Urgent Care whereby all of the core services which look after patients with an urgent healthcare need are joined up together. PHL Integrated Urgent Care work closely with system partners South Central Ambulance Service and other partners in the specific Hampshire localities offering care. The services we operate include 111 call handling, telephone clinical assessment, out of hours home visiting services and some face to face Primary Care appointments services in West Hampshire. PHL Integrated Urgent Care also operates the Lymington Urgent Treatment Centre, based at the New Forest Hospital in Lymington, seeing walk-in and pre-booked patients requiring assistance with minor illness or minor injury.

In May 2021 a newly formed Healthcare Division was created and incorporates PHLs Urgent Care, Primary Care and Youla Care Concierge Services. The new division combines PHL's healthcare operations into one team and will utilise internal and external resources efficiently to achieve our organisations vision: to be an innovative healthcare enabler, known for providing a range of high-quality care, agile solutions and cutting-edge technology service.

Job responsibilities

  • Providing definitive clinical assessment for patients accessing the service, in line with the scope of professional practice of a General Practitioner
  • Deliver clinical oversight and rapid assessment of Ambulance and ED dispositions, often assisting other clinical or call handler staff with reaching outcomes.
  • Working with operational managers to ensure the service has clear guidance for palliative care for any cases on shift.
  • Managing quickly, possible Sepsis cases.
  • They will support non-clinical operational staff to make crucial triage acuity decisions based on workloads and contractual acuity response time performance
  • Provide advice and guidance remotely to clinicians working across Hampshire and the Isle of Wight, which have called the CAS for support.
  • Act as a mentor to other clinicians and non-clinicians
  • To communicate regularly on shift with the operational shift manager
  • Contribute to shared learning communications on behalf of the medical directorate where required
  • To contribute to thematic learning opportunity through exposure to on shift challenges and solutions.
  • Take all reasonable steps to manage and promote a safe and healthy working environment which is free from discrimination.

Qualifications

Essential

  • Active registration with GMC.
  • MRCGP
  • Level 3 safeguarding of adults and children
  • Must be able to demonstrate and meet competencies of all mandatory training as required within the policies of the organisation.
  • Must be authorised to work in the UK.
  • Must be able to clear and enhanced DBS check.

Desirable

  • Experience of chronic disease management
  • Accident and emergency experience
  • Experience of telephone triage assessment and relevant pathways/electronic prescribing
