We have flexible shift opportunities in Plymouth andthe surrounding areas for nurses with current NMC Registration and competent in the management and delivery of IV Therapies.

Working in partnership with our NHS colleagues to provide individualised quality nursing care to patients requiring Parenteral Nutrition and other IV therapies in their home.

Main duties of the job

Your main responsibilities will be:

  • Providing individualised quality patient care to Homecare patients in their home setting.
  • Administering prescribed IV therapies and associated care and support to patients who are unable to be trained to independence.
  • Providing training and education to support patients/carers in the connection and disconnection of Parenteral Nutrition/to use infusion devices and any other prescribed therapies to independence.

What you'll need to succeed:

  • Registered Nurse with current NMC registration.
  • Competent in the management and delivery of IV Therapies.
  • Ability to work alone.
  • Ability to make decisions in a clinical emergency situation.
  • Current valid driving licence and your own car to travel to patients homes.
  • Flexibility to travel and visit patients outside local area.

Hours of work:

Monday to Sunday. We work split shifts due to disconnecting the patients in the morning and connecting them in the evening. Therefore, our nurses work 4 hours in the morning and 4 hours in the evening with a break in the middle. As a bank nurse, you can choose to work either AM or PM shifts.

Job responsibilities

  • Provide individualized, compassionate, quality patient care based on empathy, kindness, respect and dignity to all TransCare patients underpinned by current local and national guidelines and policies.
  • Demonstrates a pro active approach in being aware of the mental health needs of themselves, their colleagues and patients.
  • Demonstrate a flexible approach to support the nurse led service working in accordance with the nursing rota, and the Annual Leave Policy for TransCare Nurses. This includes travelling to and working outside normal territory as required, adapting and adhering to their local trust policies.
  • Support the Installation of new TransCare patients complete all relevant documentation and escalate any issues to appropriate internal and external teams.
  • Administer prescribed clinical homecare therapies and associated care and support to patients who are unable to be trained to independence, including HPN, IV, MABs, Methotrexate
  • Provide training, education and support to patients/carers in the care and management of their CVAD (Connection, disconnection, prescribed therapies & dressing change) in accordance with local policies and national guidelines to independence.
  • Report any clinical changes in patients to the relevant TC nursing team or Regional Lead Nurse or Trust.
  • Communicate in an effective and timely manner with appropriate internal and external teams ensuring strict patient confidentiality
  • Take accountability for ensuring all incidents and complaints are raised in accordance with policy, being part of root cause analysis, reflective practice, lessons learnt and future developments.
  • Maintain contemporaneous nursing care records at all times in accordance with the NMC`s record keeping guidelines, appropriately managing sensitive information.

  • Support the TC nursing team and Practice Development Nurses in co-mentoring Bank nurses.
