This vacancy is advertised on behalf of St Helens Medical Centre.

A vacancy has arisen for a Full-time Community Health Care Assistant

We are looking for an experienced HCA to join our team. You will be involved in meeting the needs of our patients in Care Homes and in their own homes. Skills in engaging with 3rd party agencies would be expected as would developing care plans.

Being trained in phlebotomy could be an advantage

Salary dependent on experience and negotiable.

Main duties of the job

To provide support to the Nurses, Nurse Practitioners and General Practitioners to ensure the efficient and effective provision of the full range of nursing services to the patients of the Practice.

Prepare treatment rooms and treatment room equipment.

To maintain adequate clinical stock levels and medical stationery.

Job responsibilities

Job Title: 

Community Healthcare Assistant

Responsible to: 

Lead Nurse/Practice Manager and GP Partners

Job Purpose:

To provide support to the Nurses, Nurse Practitioners and General Practitioners to ensure the efficient and effective provision of the full range of nursing services to the patients of the Practice.

Prepare treatment rooms and treatment room equipment.

To maintain adequate clinical stock levels and medical stationery.

Duties and Responsibilities

Clinical

1. To undertake all duties in line with the training and instruction provided by the Nurses, Nurse Practitioners and General Practitioners

2. Assistance at clinics ensuring patient care by chaperoning and assisting patients to dress and undress

3. Conduct weight and height measurements, test and record blood pressure and blood sugar levels

4. The testing and result recording of clinical specimens

5. Provide assistance in minor operations ensuring patient care by chaperoning

6. Venepuncture and the taking and preparing of pathology samples

7. Undertake ECG

8. Signpost services for both newly registered patients and complex vulnerable patients and offer wellbeing checks.

9. Carry out routine patient dressings in accordance with Practice guidelines

10. The collection of clinical waste, ensuring appropriate storage prior to disposal

11. The ongoing cleaning and sterilisation of clinical equipment and working surfaces, preparing treatment rooms prior to use

12.Ensure adequate infection control procedures in treatment and consulting rooms

Administration

13. Organising and attending Community Meetings and any other relevant meetings

14. Receiving telephone messages in the treatment room, recording and transmitting accordingly

15. Completing Care Home assessments in line with the PCN DES

16. The inputting of information onto the Practice clinical system

17. Use of the Practice clinical system for patient searches for clinics and patient recall. Completing patient reviews and patient care plans

General

18. To ensure adequate clinical stock levels and medical stationery and maintain an effective stock management system

19. Ensure adequate stock levels in stock room, treatment rooms, fridges and cupboards

20. Any other reasonable duties which may be requested from time-to-time
