This role will be based out of General Practice surgeries based in the North East PCN on the Isle of Wight. You may be asked to work from any of the 6 practices in this location and will be employed by One Wight Health
Hours: Minimum 22.5 hours per week
Salary: £13.79 per hour + NHS Pension
A full current driving licence and use of a car is essential.
NE PCN are looking for a Care Home Care Coordinator to join the team this is a new and exciting role which focuses on the needs of our patients who live in care homes.
Job Role:
To act as the liaison between the Primary Care practices and the care homes to ensure that our patients get the right support at the right time.
This role will suit a warm communicator who can also demonstrate excellent skills in record keeping and administration.
You will work under the supervision of the clinical team led by the GP who will support you in the development and on-going delivery of the role.
Main duties of the job
The key elements of this role :
Building effective relationships with all general practice colleagues
Building effective working relationships on behalf of the PCN with the Care Homes in their area.
Building relationships with other service providers such as the Trust and Adult Social Care.
Ensuring that the needs of patients and their families are central which can include developing a single personalised care plan as well as monitoring and adjusting them when needed, to ensure seamless provision of relevant services at all times in line with PCSP.
Meeting with patients, as well as any relevant family members or carers, to discuss what they need and want from their care, and what services are available to them to support their personalised care requirements, signposting and navigation to the correct support.
This role has a high need for excellent record keeping and clear paperwork so strong administrative skills are needed.
To support clinical colleagues by attending Multi-Disciplinary Team meetings and identifying patients who may need additional support.
Evaluating interventions and care plans with the care team, including identifying areas where and when further care may be required
Maintaining a good awareness of developments in the field

The area offers a mix of urban and rural areas including the major gateway towns of Ryde and East Cowes. Its population profile mirrors that of the wider population, with a higher proportion of older people. Around 22.5 per cent of people living in the North East Locality PCN have a long-term illness or disability. The locality also has the highest rate of unemployment at 4.83 per cent and contains several areas of deprivation but, conversely, parts of the area are also home to many second homeowners.

The network is well resourced both in terms of GPs and a wide range of different practitioners operating within and across the practices including advanced nurse practitioners and shared roles like paramedics, providing a patient visiting service, and clinical pharmacists. The practices also share flu clinics in the Ryde area and phlebotomy services and work to empower patients and their carers to improve their physical and mental wellbeing.
