Main duties of the job
Responsible for the clinical assessment, investigation, diagnosis &
treatment and/or referral of patients within urgent care services, identifying
complex and specialist needs and will develop, implement and evaluate
programmes/plans of care.Working as an Advanced Practitioner with a high degree of autonomy, the
post holder will manage undifferentiated & undiagnosed minor injuries &
minor illness, using advanced clinical skills within own sphere of competence. To provide expert clinical care for frail, elderly & housebound
patients with acute/intensive medical needs. To diagnose, review and treat
patients in their own home. Work collaboratively with the team & external providers, to ensure
the right care, by the right person, at the right time and in the right place.
Practitioners will aim to enable individuals to maintain or regain independence
or ensure a plan is in place for them to achieve their optimum personal
potential and to prevent unnecessary hospital admission.
Support the delivery of policy and procedures,
providing clinical leadership to achieve a quality, evidence based service,
excellence in clinical practice, this together with continuous professional
development are integral to the role and development of the service.

Essential
- Educated to masters degree level* in the relevant area of expertise so that work encompasses the four pillars of clinical practice, leadership and management, education, and research
- Advanced clinical practitioner nurses must have completed an accredited MSc advanced practice programme or the HEE e-Portfolio (supported) process
- Registered with appropriate professional regulatory body
- Relevant post registration qualification in autonomous practice or equivalent.
- Evidence of relevant post registration experience
- Independent Prescribing or evidence of working to PGDs with advanced knowledge (AHPs)
- Evidence of continuing professional development
- Excellent written and verbal communication skills
- Computer literacy
- Knowledge of primary and urgent care issues
