South Wight Medical Practice are offering a unique opportunity to work in a rural dispensing GP Practice. We are family orientated, yet innovative and are looking to recruit a forward thinking Registered Practice Nurse to complement our small but friendly team in caring for our population of 6000 patients. We are based across three sites in the villages of Brighstone, Godshill and Niton.
The ideal candidate will be a qualified Practice nurse with an interest in Asthma and COPD management or diabetes; however, we would consider experienced nurses new to general practice as training can be provided.

Main duties of the job
The Main responsibilities
of the role are normal Practice Nursing duties including:

Chronic disease management.
·
Health promotion.
·
Assist in and perform routine tasks related to
patient care.
·
Cytology.
·
Basic wound care.
·
Removal of sutures.
·
ECGs.
·
Asthma, peak flow readings, Spirometry.
·
INR monitoring using INR star.

Job responsibilities
The post holder will work as a member of the
primary healthcare team and be responsible for the delivery of general practice
nursing to the whole practice population. They will support the GP’s in
promoting excellence in care, providing an efficient and effective quality
patient care service in accordance with the GMS contract, enhanced services and
any other initiatives that may be introduced.
Job
Responsibilities:
- Assess, plan, develop, implement, and
evaluate programmes to promote health and wellbeing and prevent adverse
effects.
- Identify and manage as appropriate treatment
plans for patients at risk of developing long term conditions.
- Prioritise health problems and intervene
appropriately to assist the patient in complex urgent or emergency
situations, including initiation of effective emergency care.
- Support patients to adopt health
promotion strategies to encourage patients to live healthily and supply
principals of self-care.
- Deliver opportunistic health promotion
such as new patient medicals
- Provide information on prescribed or over
the counter medication.
- Assess and care for patients presenting
with uncomplicated wounds.
- Support and manage health needs of women
presenting for cervical cytology and those requesting information relating
to family planning needs.
- Implement and participate in vaccination,
immunisation programmes for both adults and children, advise support and
where appropriate administer vaccinations for patients travelling abroad.
- Promote and deliver evidence based care
for patients presenting with aural conditions.
- Assist Senior practitioners in providing
Minor surgery sessions.
Key Tasks:
- Wound care/ulcer care/Doppler
- Removal of Sutures
- Urinalysis
- ECG’s
- Chronic Disease management
- Routine immunisations & Childhood
vaccinations
- Chaperoning and assisting patients where
appropriate who are being examined by another clinician
- Assisting GPs with minor surgery and coil
fittings
- Requesting pathology tests, for example
urine culture, swabs
- Following agreed clinical protocols with
referral to Lead nurse or GPs as appropriate
Other tasks will include:
- Maintaining and cleaning equipment and
rooms used by the nurses and GPs. Stocking and rotating items as required.
- Ordering of Health Promotion Literature
and display appropriately
- Ordering of appropriate stock
- Ordering of vaccinations to maintain stock
levels
- Participation in administrative systems
in the Practice
- At all times there will be a need to
maintain accurate records
- A duty to advise Lead nurse of potential
problems or errors within the range of assigned tasks
- Attend and participate in any Practice
meetings when required.
- On occasion there may be a requirement to
undertake home visits
- Any other responsibilities within the
scope of the grade that may be agreed from time to time.
