Job summary

An exciting opportunity has arisen for a GPhC Pharmacy Technician to join the Northolt Greenford and Perivale (NGP) PCN, working as part of the pharmacy clinical team.

The PCN consists of 11 practices covering around 75,000 patients. We have been involved in supervising and mentoring clinical pharmacists since 2015 and have already developed an excellent team of 13 Clinical Pharmacists and 4 Pharmacy Technicians across our practices.

As a pharmacy technician you will work alongside our clinical pharmacists contributing to the management of patient care and the high quality, safety, and value by supporting medicines optimisation and prescribing processes. This includes supporting development of processes and system to deliver pharmaceutical and medicines optimisation services and contributing to the management of long-term conditions.

Main duties of the job

The ideal candidate will be an enthusiastic and motivated pharmacy technician.

You should be an excellent communicator, computer literate able to work both independently and as part of a multi-disciplinary team in a diverse environment.

You will have a passion for working as part of a wider practice team and with our patients to ensure they get the most benefit from their medicines. The post is based in NGP PCN in Ealing and travel across the locality will be required.

About us

The clinical pharmacy team is an established team that is continually evolving to meet the needs of our PCN population. We have excellent relationships with our GP Practices, the ICB, Ealing Borough locality teams and local acute and community trusts.

Now we are looking to offer pharmacy technicians the opportunity to develop and progress in the primary care setting. We offer the opportunity for training, which supports delivery of the NHS Long Term Plan and PCN services.

The team are keen to support local healthcare professionals with all medicines related aspects but more importantly support and harness medicines use to benefit the local population.

You will be employed by Ealing GP Federation who has supported and developed primary care services for the population of Ealing since 2014.

Duties

Clinical

Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.

Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.

As determined by the PCN, support medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacies

Support the Clinical Pharmacist in Structure Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR

Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.

Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.

Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.

Technical and Administrative

Support the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed.

Implement efficient ordering and return processes and reducing medication wastage.

Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).

Liaising with patients and GPs and Clinical Pharmacists in both practices and community pharmacies about placing appropriate patients onto repeat dispensing prescriptions and explaining the process and benefits

Develop relationships with other pharmacy professionals and members of the multidisciplinary team to support integration across health and social care including primary care, community pharmacy, secondary care and mental health.

Support practice reception teams in streaming general prescription requests, so as to allow GPs and clinical pharmacists to review the more clinically complex requests.

Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.

Support the PCN to deliver on QIPP agenda, QOF and locally commissioned enhanced services

Support the PCN in reviewing and developing practice policies for CQC requirements

Collaborative Working Relationships

The post-holder will be required to work with a wide range of stakeholders. It is important that the post-holder:
