Job summary
The main purpose of the job is act independently to manage a
caseload of patients referred to Discharge to Assess (D2A) beds, and to provide
full comprehensive assessment, diagnosis, treatment/appropriate management,
onward referral and discharge to this complex range of patients. The supporting
admin team with ensure the most up to date patient list is provided.
The post holder
will also be required to manage a caseload and deal
with a wide range of health needs in a primary care setting, ensuring the
highest standards of care for all registered and temporary patients.

The post holder will work independently as the named GP for ATP Discharge
to Assess (D2A) beds.
Be responsible for managing a caseload of D2A patients;
Support with the assessment, care-planning and on-going clinical care
needs for patients, including through a mix of home round visits and remote
support.
Complete RESPECT forms and/or advanced care plans where needed.

In accordance with the Practice timetable, as agreed, the post-holder
will make themselves available to undertake a variety of duties including surgery
consultations, telephone consultations and queries, visiting patients at home,
checking and signing repeat prescriptions and dealing with queries, paperwork,
reports and correspondence in a timely fashion.
Screening patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current Practice disease
management protocols, developing care plans for health.
Admitting or discharging patients to and from the caseload and referring
to other care providers as appropriate.
In general the post-holder will be expected to undertake all the normal
duties and responsibilities associated with a GP working within primary care.
