The post holder will work as part of a team of Wellness Coaches that already deliver the SmokeFreeLife Berkshire service to diverse communities across Berkshire. As a key member of the team, the post holder will be expected to support colleagues in the set-up, delivery, monitoring and evaluation of the NHS Health Check Programme in West Berkshire
The post holder will be expected to support the delivery of NHS Health Checks in West Berkshire to clients who live in or are registered with a GP in West Berkshire.
Main duties of the job
NHS Health Check
Support the delivery of the service by triaging, assessing, and empowering people who might be at a risk of developing heart disease by enabling and motivating them to address lifestyle issues such as: stopping smoking, achieving a healthy weight, increasing physical activity and drinking too much alcohol.
Undertake NHS health checks and general health assessments using the Integrated Health Systems (IHS) in multiple local community settings, in order to support people to make better lifestyle choices and take control of their health, with the option of referring them onto a local lifestyle service as appropriate.
Work closely with GPs and other health professionals, community and voluntary organisations to ensure maximum reach to the residents of West Berkshire
Additional responsibilities
Assess and interpret clinical information regarding referred patients, in respect of all programmes, whilst maintaining confidentiality and data protection.
Identify realistic goals, monitor and evaluate progression whilst continually motivating to increase adherence to the programme.
Interact with referred patients, recording details, monitoring adherence and providing individual support and advice.
Ensure that all sessions and activities are monitored in line with agreed targets.
Maintain and regularly update patient information by providing data both electronically and in paper format to contribute to the timely monitoring and evaluation process.
Receive and provide sensitive information to and from healthcare professionals, including primary & secondary care.
Motivate, support and reassure patients, whilst being considerate of and understanding barriers to change.
Establish good rapport and relationships with patients to maintain participation and involvement in all programmes.
Communicate effectively and interact collaboratively with patients, partners and stakeholders to resolve issues or problems arising.
Demonstrate flexibility in service delivery and be willing to deliver services virtually when required.
Have a flexible approach to working hours in order to meet the needs of the service.
Maintain accurate records and information systems in line with Solutions 4 Health policies.
Assist the Project Manager(s) in monitoring, evaluating and reporting on the effectiveness of programme interventions.
Establish and maintain health and safety standards within all programmes, ensuring that all activities are risk assessed and undertaken with due regard to health and safety regulations.The post holder may be required to wear a branded shirt and ID Badge at all times whilst on duty.
Additional Requirements
The post holder will have experience of: working with hard to reach and vulnerable groups; motivating/interacting with individuals with multiple risk factors; and reacting and supporting them appropriately through the programmes delivered.
The post holder will be required to deliver efficient and effective services, provide written and verbal reports, have good organisation skills, have an awareness of data protection and patient confidentiality, have experience of working to tight deadlines and targets, excellent time management skills, an ability to work independently and as part of a team, and have the ability to collect data and assist with monitoring and evaluation.
The post holder will be self-motivated, enthusiastic, and friendly, have a flexible approach to working hours and have the ability to travel across Berkshire.
Information Governance and Information Management
To operate and update a range of databases and computerised surveillance systems. Inputs data and information accurately and completely, uses the correct formats and is consistent with legislation, policies and procedures.
