Job summary

The Healthy Child Programme 4 Barnet are looking for motivated NMC registered community nurses to join our dynamic and innovative integrated team of Public Health Nurses (Health Visitors and School Nurses) delivering the Healthy Child Programme in Barnet. You will be working with children and their families aged 0-19 and up to 25 years for children with special educational needs and/or disabilities (SEND), delivering a public health service that is tailored to meet the needs of the local population.

With support from senior member of the team, the Community Staff Nurse will assist in the development of the service and work with colleagues to ensure the delivery of a safe and effective universal health provision for children and young people attending schools in Barnet

Main duties of the job

To work within the Health Visiting or School Nursing teams in the delivery of the Healthy Child Programme.

To work in partnership with children, young people and their families in empowering them to identify their own health needs and developing a plan to meet those needs.

To provide support for identified health needs at community level, in relation to national and local agendas.

To work in collaboration with other health care and service providers, including Early Health Hubs, Children Centres and Early Years settings to identify and address health needs.

To undertake and initiate health promotion and disease prevention activities to improve the health outcomes and wellbeing for children and families.

To liaise with the Health Visiting Lead where service delivery falls outside of the scope of practice.

To be able to identify children where there are safeguarding concerns, reporting to the Health Visitor or School Nurse.

To maintain accurate records of all contacts with children, young people and families in both community and home settings as outlined in NMC guidance.

To attend and contribute to multidisciplinary meetings as appropriate.

To work flexibly to meet the needs of the service. This will include some evening and occasional weekend work.
