Job summary

Main duties of the job

Lead on delivery of the Healthy Child Programme across the community of Barnet.

To work in partnership with parents, carers, children, young people, Childrens Centres, Childrens Social Care, Primary Care, the voluntary sector, and other members of the multiagency team in delivering public health services that are tailored to meet the needs of individuals and the community.

To work in collaboration with health partners and other agencies to implement the Barnet Early Help Strategy.

To ensure that children that are identified as SEND and those with complex health needs and their families receive early intervention and support from the wider multi-agency team.

To identify vulnerable children and adults at risk, initiating safeguarding and child protection procedures in accordance with Solutions 4 Health and the Barnet Safeguarding Children Partnership and Safeguarding Adults Board policies and procedures.

To attend safeguarding statutory meetings, writing case conference reports and Court reports.

To ensure that the SystmOne electronic records are updated contemporaneously and that records are of a high quality.

To work flexibly to meet the needs of the service.

To be innovative and adaptable to different approaches to delivering the service.

The Health Visitor working within Solutions 4 Health will act in a way which demonstrates our core values, mission statement and vision by putting them into practice with service users, their friends, family and carers, other staff members and multi-agency partners.

Main Duties

The specific accountabilities of this role will be flexible and will change to meet the needs of the organisation.

  • Deliver the Healthy Child Programme
  • Search for health needs in the community, identifying need in relation to national and local agendas
  • Act as a health link for other agencies e.g. voluntary services, public and private agencies
  • Identify vulnerable families and mobilise appropriate resources
  • Carry out specific health and developmental screening where identified, or delegate responsibility where appropriate to other team members
  • Work flexibly to meet the needs of the local population - this will include some evening and occasional weekend work.

Personal Specification

We are looking for a Health Visitor withexperienceof coordinating clinical staff andworking in integrated teams. They will haveknowledge of theHealthy Child Program (DH 2009) andSchool Nursing.

          Required Skills/Qualifications

          The Applicant will haveaRegistered Nurse or SCPHN qualification and a Teaching/Mentorship qualification with evidence of ongoing learning & professional development.
