Job summary

Note: The position will be based in either or across the OIdham, Rochdale or Manchester localities, hours to be negotiated.

Focused Care is a model developed in Primary Care which supports vulnerable and complex patients and GP teams in areas of high social deprivation. Taking a whole household, person-centred approach you will have a caseload, based on the practice list of all ages and conditions, with the aim to help unpick problems which hold back a person's health and social wellbeing.

Focused Care is a highly rewarding role. It aims to "make the invisible visible", advocating for patients where they struggle to engage with the system, and to support GP teams to bring evidenced lasting health changes for households and families. The role allows you to be risk aware but not risk averse and be innovative in crossing boundaries across health and social care to get the best for patients.

Applicants must be compassionate and experienced in supporting people who find themselves in complex situations. You must be able to work autonomously, with an ability to be accountable and work in a team across an MDT.

Main duties of the job

This role involves holding and managing a caseload of patients and families who need additional support and holistic intervention.

Due to the depth of working you will need to have a background in a relevant health or social field, be it a nurse, health visitor, social worker, probation worker, youth worker background. The qualification is less important than demonstrating experience in face-to-face working, desire and ability to work autonomously, safely, and with a huge passion to bring change and social justice.

Applications should be trustworthy, non-judgemental, be able to problem solve and work with a wide range of people for the good of the patient, with a can do practical attitude.

You should be able to manage your own diary and caseload and work innovatively for a positive outcome. This role is based out of GP practices, and so you will need to have a clear understanding of patient confidentiality and GDPR, and have a reasonable level or understanding of note and date recording.

Focused Care CIC is a non-profit organisation, passionate about reducing health inequalities and empowering people to take responsibility for their own health. We have a diverse, enthusiastic and collaborative team, both on the frontline and in administration.

Day to day you will work as part of an allocated GP practice, who provide another layer of support and team working. Our practitioners are part of a local Focused Care Hub. Your Hub Lead will meet with you regularly to offer guidance and direction, and you will meet regularly with your local colleagues for peer support. We are keen to support our workers in ongoing professional development.

Wellbeing and workforce resilience is valued at Focused Care and so we offer regular 1-2-1 clinical supervision. Focused Care runs a detailed induction programme with a mix of face-to-face and on the job shadowing so that you can understand the ethos, vision and values of the role, with ongoing training and appraisals throughout the year.
