Job summary

We are looking to recruit a motivated individual to take this opportunity for an exciting new position within the Ellern Mede Group.

As an apprentice social worker you will be based at Ellern Mede Moorgate undertaking the duties of an assistant social worker while studying to qualify as a qualified social worker.

Applicants must have evidence of GCSE Maths and English at Grade C or above, or equivalent.

The role is full time, 20% of which will be dedicated to study during term times as laid out by the University

You will be able to study at degree level and be expected to undertake 3 placements which will include both Ellern Mede and external services during the 3 year course.

Essential duties include:

  • Safeguarding
  • Assessments of and meeting with carers and families.
  • Liaison with community services to secure transfer of care as required.
  • To act as link between inpatient services and receiving community teams
  • Assisting with discharge planning.
  • Liaising with advocacy services.
  • Supporting patient rights.
  • Assisting with the completion of relevant forms
  • Completion of relevant applications for support. Education and work

Job responsibilities

While undertaking the apprenticeship scheme, the post holder is required to work effectively within the MDT and hold strong risk management and safeguarding knowledge/skills. Excellent working knowledge of the mental health act 1983/2007 as well as other relevant statutory legislation child protection and protection of vulnerable adults is required. Experience of working with children and young adults within a mental health setting is advantageous.

The role is within an inpatient service but liaison with community teams and understanding of community mental health services will be necessary.

During the apprenticeship program you will be required to follow the directions and policies of the University

Essential duties include:

Specific tasks Include

  • To assist in the completion of assessments of client social care needs and of carers and participate as part of the MDT within the care programme approach processes.
  • Attendance at MDT meetings and update the team on progress from a social care perspective on behalf of the social work when necessary.
  • To provide day to day support to patients as directed by the social work team.
  • To liaise with and arrange advocacy as required.
  • To provide advice and support to patients, carers and nursing colleagues around welfare benefits and financial assessments with the support of the social worker.
  • Establish and maintain effective links and working relationship with key internal and external colleagues
  • Ensure records and information systems are kept accurately including the inputting and updating of information on Care notes.
  • To act in a professional manner at all times, attend all mandatory training, keep abreast of changes in legislation, maintain CPD, maintain and act within the boundaries of your role.
  • To inform managers of individual case issues which may impact on the care and treatment of patients in the hospitals.
  • To adhere to policies and procedures of the university and Ellern Mede.
