As PCN
Care Coordinator you will work as a key part of the primary care network (PCN) multi-disciplinary
team. You will ensure care is seamless and that everyone involved is working
together. You will provide the capacity and expertise to support patients in
preparing for or in following-up clinical conversations they have with PCN
primary care professionals.
Main duties of the job
Operational
· Proactively engage with
care homes under the DHPL model.
· Liaise with care home
staff to ensure each resident has a personalised care plan.
· Work closely with
clinicians from the DHPL care home team to review patients.
· Organise and
co-ordinate referrals as appropriate, generate ICE forms as requested by
clinicians.
· Liaise with other teams
within the wider system as appropriate to ensure holistic care is provided to
all patients.

Primary Care Networks (PCNs), supported by Dacorum Healthcare Providers Limited
