Job summary
An
exciting opportunity has arisen for Central Manchester Networks ltd to recruit a
Social Prescriber to be based within Ardwick & Longsight PCN. A&L PCN
is a group of GP practices working across Central Manchester. Primary care
networks think about the wider health of their population, taking a proactive
approach to managing population health. By better understanding their wider
communities, Primary Care Networks are able to focus on preventative care by
tackling health inequalities and identifying those who would benefit from
targeted, proactive support.
Main duties of the job
The
purpose of this flexible and exciting role is to support in delivering a
bespoke community-based dietetic programme, which best meets the needs of A&L
PCN service-users.
Deliver a programme
of support which meets the unique and diverse needs of service-users
from across the PC. The programme Real Food
Real Health encompasses a person centred, strength-based approach to helping
the community to understand about their diet and how this impacts their health.
Deliver
services in a variety of settings, including but not limited to: community
clinics, GP surgeries, community venues, and group
settings.
Provide
personalised support to individuals, their families and carers to take control
of their health and wellbeing, live independently and improve their health
access and outcomes
Develop
trusting relationships by giving people time and focus on what matters to me.
Take a holistic approach, based on the persons priorities and the wider
determinants of health. Co-produce a simple personalised care and support plan
to improve health and wellbeing, introducing or reconnecting people to
appropriate community groups and statutory services.
Work
with a diverse range of people and communities, to draw on and increase the
strengths and capacities of local communities, enabling local organisations and
community groups (including faith groups) to support the social prescribing service

PCN Resource and Advocacy:Support the GP practices to refer patients to the Real Food Real Health ProgrammeDevelop and sustain excellent working relationships with the practice teams within the PCN, working in a way that is collaborative and informed.Promote social prescribing, its role in self-management, addressing health inequalities and the wider determinants of health.As part of the PCN multi-disciplinary team, build relationships with staff in GP practices within the local PCN.Be proactive in developing strong links with all local agencies to encourage referrals, recognising what they need to be confident in the service to make appropriate referrals.Plan, adapting and develop the service to deliver interventions which best meet the needs of the cohort you are supporting.Work with community services and assets to deliver a strengths-based, community-led approach.Maintain thorough and secure patient records. Collating data and evidence to support research and evaluation of the service.Meet people on a one-to-one basis, making home visits where appropriate within organisations policies and procedures. Give people time to tell their stories and focus on what matters to me. Build trust and respect with the person, providing non-judgemental and non-discriminatory support, respecting diversity and lifestyle choices. Work from a strength-based approach focusing on a persons assets.Be a friendly and engaging source of information about health, wellbeing and prevention approaches.General admin tasks including:
Managing spreadsheets used for triaging referrals and booking people on to various groups including troubleshooting
- Ensuring these are kept up-to-date with only relevant information stored on them
Helping managing forms used for signing up to courses and receiving feedback including troubleshooting
- Pass feedback on to team and record appropriately for use in case studies and future planning and development
Phone and email communication with patients throughout booking on and group running process, occasionally clinicians and other community partners and liaison.
- Responding in a timely, professional manner
- Helping draft, design and store communication templates
Managing email inbox, responding promptly to emails and liaising with patients, community partners, clinicians and all other queries.
